I so appreciate you do this work for us. It keeps us out of the marketplace of non-solutions. We only have so many resources and so much time. We can't spend it on things that are specious are dead ends. Time spent on your videos on the other hand is time incredibly well spent.

As one who has done electrical work in my engineering career, I can confirm that getting electrocuted helps you focus in direct proportion to the number of volts you recieve though it does have diminishing returns above a certain voltage threshold... in fact, the mere risk helps one focus. I cant see how this could actually work though. Would be nice however, Id make them take my money in a heartbeat if I could remember where I left my wallet.

A similar looking product, the Fisher Wallace Stimulator device SEEMED to have worked for my depression for a few months and slowly dicipated to how I was feeling before , reapplications of the device for the same period of time did nothing.

Thank you (cool) grandpa Russ. I went through both TDCS & neurofeedback under a QEEG specialized neurophysiologist. 1. The primary takeaway for me looking at TDCS research and NF both is that QEEG is more or less mandatory. Otherwise it's like dynamite fishing, and the literature reflects that. 2. Between the two I find NF to be more helpful as it's a weekly treatment which encapsulates more effects. Daily TDCS is exhausting. Both helped me tremendously with ADHD and especially PDD. That being said I still lack the dopa juice and medication is still relevant for that. 3. Either can be and should absolutely not be independently used by regular people to treat themselves. The reasons are numerous and cannot be understated. Experts in the field are very few. Richard Soutar comes to mind, over at New Mind. The traumatologist Bessel Van Der Kolk reps it as well. Me and my therapist are from Israel - She learned this in Europe. Wishing well

Thank you so much for compiling all this information and sharing it with us! It's just... very despairing as someone who doesn't respond to stimulant medication. Seeing that clarity when I trial various stimulants is just... soul crushing. The light at the end of the tunnel burns so bright when I trial a stimulant medication. I'm able to start tasks because I need to do it... I'm able to properly prioritize when cleaning, my threshold for sensory overload is significantly higher. I'm able to be super involved with my kids from both being able to focus and being able to get the household tasks needed to be done. Then after a few days to a few weeks, the rug will be unceremoniously ripped out from under me again, and I'm back to only a mild improvement of my symptoms compared to being unmedicated. I'm incredibly thankful Qelbree is able to do some decent heavylifting for me, but I wish that there was something that could actually do significant symptom reduction for me like other people get to experience. I was super hopeful this might finally be the missing piece, but instead, it's just back to hoping some other miracle like Qelbree will pop up down the line.

8:58 That’s 1-5 sessions, which is less than 2 weeks. That is no more helpful than saying 7 days of atomextine doesn’t produce a good improvement. Even the manufacturers don’t claim that it works in 5 sessions.

interesting stuff. considering tDCS has been around for quite some time (going back to DARPA work in 2008) I'm surprised no one started doing studies before now. the results at DARPA for accelerated learning were impressive; it makes me wonder if our ADHD brains and their glitchy dopamine regulation would have an impact there. The early DARPA work didn't separate test subjects into neurotypical vs neurodivergent, but knowing that the military will not enlist those on stimulant medication it's likely there was an automatic "weeding out" of people with ADHD who would choose to keep medication treatment over enlisting. I know you were focused on the touting of this device as a treatment and thus excluded TMS, which must be performed in-clinic and generally has a minimum treatment period of 6 weeks. I'm curious if there have been any studies on it, though, even if they were knock-on effects - keeping in mind that many people with ADHD suffer with medication-resistant depression and anxiety and could have sought out TMS for relief of those symptoms.

There are plenty of studies prior to 2020, including studies prior to 2010. The reason he didn't cover those is that they would have been captured by the 2020 and later systematic reviews and meta-analyses, which are considered one of the strongest forms of evidence as they analyze prior research and evaluate their methodology, statistical power, and researcher bias, (etc.) as well as taking the results from those studies and using them to synthesize a meta result.

The finding from the Westwood et al. (2021) article has piqued my interest regarding the finding on processing speed. I wonder if there might be applications for cognitive disengagement syndrome? I hope someone does that research.

I did 36 sessions of TMS for my depression/anxiety etc . Not a headband, I went to a real doctors office and sat in a chair lol I’d be weary of this product , at least for now. I used the Neurostar TMS machine btw

I thought so. We wouldn't be looking at these devices under a microscope for 10 years trying to prove that there is SOME effect if they really worked wonders. People would instinctively be reaching out for them like they go for self-medication or exercise or whatever. Those big business marketing guys would just love a non-regulated device that they could sell us at whatever price. Throw in a subscription model and some celebrity endorsements and you have the big bilions. SMH

What about TMS for ADHD, is it the same? Is ADHD a contraindication for this treatment? Two years ago, the university hospital I was working in was starting a double blind placebo study on TMS for depression- the first in my country, we’re a bit slow on this front. I was excited both as someone interested in the subject but also bc I’ve had depression since I was abt 14 and tried everything apart from ECT and a lobotomy at this point lol. I had, at that time, been recently diagnosed with ADHD and started Vyanse, which has been the best treatment so far in that it’s actually alleviated my depression a bit, but I still have dysthymia and depressive episodes and I’ve tried various medication with uncertain effect. Anyway, I was luckily accepted as a possible candidate for the study and fulfilled all the criteria- but then I mentioned my ADHD and it turns out that was a contraindication for the study. I was bummed ofc, bc after the study the participants that got placebo was gonna be offered TMS sessions and this isn’t available as a treatment otherwise. I understand from a study pov that I wasn’t included since they’d wanna rule out as many sources of error and that ADHD could be one, but I started to wonder if TMS for ADHD was a contraindication in general and if TMS was a treatment form commonly used in other countries for ADHD patients with depression (or depressed patients with ADHD). I was also a bit annoyed to find out that study I was kicked out of was focusing on improvement in executive dysfunction in depression - since this is something I and a lot of us with ADHD struggle with too😅 Thanks for reading and thank you Dr Russell for this channel and updating us all on science and studies!

So, for now, it looks like TDCS isn't very helpful. If you could look forward 10 years, in what area do you predict there will be the biggest improvement in ADHD treatment, Dr. B? Tweaks to existing medications? Entirely new classes of drugs? Surgery? Medical implants?

I'm not Dr. Barkley, but it's not that tDCS is very helpful, but it more has to do with we don't have any certainty as of now to determine whether tDCS is sufficiently helpful for ADHD due to lack of research. And, I don't think any of the alternatives you listed is going to see any BIG improvements to supersede over our current meds/therapies because 10 years is hell of short period for researchers to confirm their study.

So, the device does treat ADHD, just not in any meaningful way:) But most of the world currently doesn't have access to proper treatment, and (in part) the company is pivoting on the (in)accessibility. I might be one of the few who is open, if not outright constrained, to self-medication, only as long as it's done right -- to the extent that's not an oxymoron. This device doesn't seem right. Do you think you can review l-theanine/caffeine combination studies as potential treatment for ADHD?
