something is wrong when someone cares so much about criticizing the person instead of the idea. The idea that childhood adversity may play a role in the development of attention deficit disorder is not new, Maté did not invent this idea. He, nor anyone else, says that trauma causes attention deficit, just that developing in a stressful environment can be a trigger and activate the genes associated with this disorder. just read Robert sapolsky to learn about the criticisms of twin testing, adoption studies, etc., how can anyone still use this as an argument; to discover that there are several genes associated with certain personality or behavioral characteristics, some from the sphere of attention deficit, but only in the presence of maltreatment or negligent (or distracted) parenting and to understand the difference between being heritable and having a high degree of heredity... and it's a shame because I love your videos about the importance of emotional dysregulation in attention deficit. Now, isn't it true that the presence of symptoms of emotional dysregulation is also associated with growing up in unpredictable, poorly treating or negligent environments? and emotional dysregulation does not just mean impulsivity, repression of emotions is also emotional dysregulation. In clinical practice we realize perfectly that the symptoms of attention deficit can be understood as adaptations to the environment. and the clinical presentation of attention deficit is different in each patient, of course, Sapolsky says that there are many paths to reach the same destination (in this case the diagnosis). and regarding the culpability of parents, Maté himself says that this doesn't make sense because we don't raise children in isolation from society. Today's society itself means that the conditions we have to raise our children are such that even if we are careful, loving, etc., we as parents are subject to stress, which will influence the baby, Bolwby and Winnicot were already talking about this. So I don't see why people should so vehemently attack someone who's theory isn't even new. just read authors like Coimbra de Matos or Arno Gruen for example. Do you really want us to believe that a child's environment does not influence their neurodevelopment? As sapolsky says: the environment influence starts since we have an environment this means the uterus, don't we know already stressed during pregnancy plays a role in many mental health issues to... Everything has to be considered when we try to understand wy someone developed ADHD...

Thought I might have hypophantasia (not absent, but weak), but took a test online for it and it suggests I'm average. Not sure if the two disorders are linked, but I will say this. I don't particularly *enjoy* trying to imagine things because even if I can conjure the imagery on demand, it requires too much focus to maintain more than a flash or a hint of what I was looking for. Which could definitely impact working memory.

REMINDERS ABOUT TIME ARE NOT HELPFUL FOR THIS CONDITION. I just feel like this needed to be repeated. It is so important and annoying when you are trying to get something done being reminded of the time that you have left.

As I have often said, you are a Godsend to those of us dealing with ADHD/ADD. I am a Biblical Counselor and I specialize in Marriage and family with an emphasis on children and adults with Neurodivergent disorders. I have several Counselees who are taking Bupropion. Most of them say that they can focus better and sleep better than when taking stimulants. Do you have any information on this, and if so could you speak about it?

There is enough research now to prove that it is effective but the manufacturer does not intend to get FDA approval for use in ADHD. Nonetheless, it can still be prescribed off label by a physician for ADHD. The drug does target dopamine like stimulants but also other neurotransmitters and so also seems helpful when anxiety or depression are comorbid conditions. Just took my test. This is super interesting and I could totally see it. I was hyperfocused during the test, I’m sure I did well, but my executive functioning is fucked up. This is great research and mirrors my subjective experience well. We’ll see what my results are.

Still interesting. I cant help but feel like this could be diet related since stimulants tend to discourage hunger. Quality diet is already difficult to attain these days. Regardless, people would probably still do better to do their best to keep up with good diet, regular exercise, and quality sleep while on these medications.

I've gotten many everyday or even work tasks down to a science but it's the variables that add up and can cause a well planned set of appointments to fall apart In addition ,it seems the vast majority of others will cut corners on tasks and ." cheat" or are able to ignore requests,say " no" or are woefullly/ or willfully incompetent Does pefectionism overlap with this?

Adderall worked great for me but my doctor took me off it and wanted me to take Strattera but the thing with Strattera is you can't cold turkey it without getting physical withdrawals whereas with Adderall I can cold turkey it with no physical withdrawals I'm just tired for a couple days, I'm not willing to do those kinds of meds anymore been through withdrawals more than once because a doctor decides to take it away or want to change the med to something else or I lost my insurance and couldn't afford the medicine and now they are having a medication shortage so what happens when a person can't get their medicine??? There is going to be ALOT of people going through withdrawals and they can be brutal, I'm not going through that NO MORE!!!!!!!!

ADHD is not real. It is just a made up diagnosis handed out by quacks to people to hide behind so they don't have to deal with life on life's terms and to validate their need of victim status. ADHD is just another in the long line of made up conditions and ailments handed out like candy to people who need their feelgoods stroked and are seeking attention because they refuse to conform to normal societal rules and requirements and make money for fake "doctors" and Big Pharma. All of you need to get over yourselves and accept and live life on life's terms.

I am 34 years old, I already know about aphantasia because I realized a few years ago that despite dreaming with images, when I try to form images in my mind consciously I encounter enormous difficulty, or I simply can only create an internal dialogue... I was diagnosed with ADHD recently and I have also been looking into the possible implications that these two conditions may have on each other. I read that mental imagery requires initial processing in the frontal and parietal cortex before recruiting other areas of the brain

I know you are an amazing expert on ADHD but that list of creative thinking has a huge overlap with myself and my ADHD friends. Of course we are almost all graduates of some graduate program with MS or PhDs so we clearly don't represent the whole spectrum of people with ADHD. However other people I meet with ADHD often have these characteristics but lack the follow through ability that higher intelligence helps overcome the ADHD defico.

It's so upsetting that my life is completely derailed since lapses in treatment caused by the pandemic. Sleep deprivation, brain damage etc. I'll probably never be able to function again. My life was going so well. But ill probably never be able to adequately treat it again since.

I’ve been trying meditation to help with my ADHD. I think I’m in the group that had adverse effects. While I felt better and was more relaxed, my focus and memory got WORSE! The next day, I left my car running while I was at the grocery store. I was so zoned out that I forgot to turn off the engine. Because of that, I’m leery to try it again.

On the first finding regarding an increase in instances of TBI in children with ADHD and an increase in siblings compared to the population, I am reminded of something you once mentioned in a lecture regarding structural brain development and how structures like the cerebellum or basal ganglia have a genetic basis to it. Is it possible that this may be a contributor to the instances of subclinical symptoms as it pertains to the cerebellum and basal ganglia's role in balance, coordination, motor control, procedural memory, and proprioception? I also wonder if the undertaking of riskier physical behaviors of children with ADHD is adopted by siblings as they interact with each other in the play environment. Just some thoughts.

Could it be that people with ADHD are less likely to be hospitalised for TBI than their siblings because they are less likely to seek treatment?

I have ADHD and aphantasia of the diminished capacity type, rather than complete inability to generate mental images. Curiously enough, I *can* very clearly generate mental sounds. I grew up in a family of musicians and artists, so perhaps growing up in a home filled with music contributed to that ability. If I think of a sound or a song I can practically hear it in my mind. But trying to imagine even something as simple as an apple, what is conjured in my mind seems more like a flattened concept of an apple lacking detail rather than a detailed 3D version of the fruit that I could turn around to examine from various angles. Like I am one of the folks trapped in Plato's allegorical cave, stuck with mere shadows of the actual thing. When it comes to taste or touch I cannot generate anything at all.

Dr Barkley, Thank you so much for creating your channel. I am 68 and I just realized reading your book "Taking Charge of Adult ADHD” that most likely I have ADHD. It explains evens of my like, interactions and why it was so hard for me to survive, despite the fact that I have an IQ of 141 and graduated University as well. I found huge relief finding out the fact that I probably have ADHD tah helps me not to be ashamed and not to blame myself harshly. I currently live ina small down of a small country and there is no way to be professionally diagnosed. Thank you from the bottom of my heart for your exellent book on ADHD.

On excessive acquisition… I know personally, there are many times that I’m not actively going out and acquiring things. A lot of things are given to me. For example, my mom (trying to be helpful and thinking I doing wear enough different clothes) will pick up pick clothing that she thinks I would like when she’s out and give it to me). This also happens with other Knick knacks and even recently it’s been happening at work. But the problem is that even when they are things that I know I don’t want, I find it hard to say no (especially if I doing say anything right away). But further to that, I feel bad that I now have this perfectly good piece of clothing that I do not want, throwing it away feels morally wrong, and being organized enough to donate it feels impossible!

For the first study it sounds as if they have found a result of increased child abuse, impacting both adhd children and non adhd children in the family.

Presumably a parent of the adhd children would also likely have adhd, which may explain childhood accidents, too?

I am wondering how TBI looks in countries other than China, with differences in the amount of automobile ownership and similar. Americans have four times the amount of per capita automobile ownership. I am also seeing statistics saying Americans just have a lot more TBIs in general compared to the Chinese - this both could be a data tracking issue, but also an impact of automotive ownership and the like. So, just generally, this might be underestimating how often TBI comes with ADHD, by a lot, in other countries with different cultures and finances.

When imagining something in minds eye - is the image a ‘real’ thing inside your head that is stable and that you can kinda ‘look’ at, explore, change up and see it clearly? Or is it a a fleeting, hard to create/grasp/see/keep vague or piecemeal image that feels like it is wisp of wind blowing in and away from hard to reach crevice of the mind? I’m guessing I have aphantasia because I’m asking this question :/ (adult diagnosed ADHD)

Any psychiatrist, psychologist, therapist, or mental health professional should adopt this as a common practice. Whether the patient chooses medication, therapy, or both, the patient (especially adults) or their parent should be warned beforehand that they will be confronted with a reality that is substantially different than the one they remember prior to diagnosis and treatment, which can be very emotionally jarring. Not only is there a recognition and reconciliation/reckoning with past events we were previously unaware were caused by ADHD, but the new understanding of the present and a redefining of the future. It is an entirely new version of the patient’s personal narrative. It wouldn’t have change my mind about treatment, but would have at least prepared me for the emotional roller coaster and associated thoughts that ADHD treatment introduces. I explained this to my treatment team and they’re considering adjusting their practice because of it. I’m definitely going to show them this video. Thank you!

In my experience when I got a TBI I got rejected and couldnt advocate against it well enough on my own.

Based on watching my children (one diagnosed, one is only 4 and shows traits but so far does not show impairment) even if the sibling of the diagnosed ADHD child is symptom free they are likely to get a TBI due to their sibling’s impulsive behavior around and toward them.
