The Holy Knights were first mentioned right after Wano when Akainu was talking to one of his vice admirals about all the incidents that were taking place after the Reverie. They mentioned that the Holy Knights intervened in the deliberation of what happened between St.Charlos and St.mjosgard (the one who attempted to kidnap Shirahoshi and the one that ended up helping her and Vivi).

Im honestly not mad about the pacing, i think the reason they are doing it is because they are so close to the manga at this point. But they are not really adding filler at all, just going more deep into things that were short/off screen in the manga which im happy with.

I'm not sure which character mentioned, I believe it was Cosaron on Dessrosa acr, or professor Clover on Robin's past, but the Tenryubitos are the descendants from the original royal families who created the World Goverment Then they decided to live in Marieyoa so new Royal families appeared in each country. That's why Doflamingo chose to rule Dessrosa. The Donquixote family was the original rulers for that country.

If the Gorosei bow to Emu, he is the ruler of the world government even though the rest of the world does not know about it. Most of the world does not know much about world government, and that's how they can maintain their authority.
