2m+ tall basketball player - modified leg position works well, otherwise his treatment becomes my workout 😀. Not to mention I can barely reach the PSIS area otherwise. Thanks John!

Wanted to get your insight John - my wife has severe and "random" shooting pain down her right hip, thigh and into her knee. When she has this pain, 95% of the time she has an anterior rotation on her LEFT side and I do a Chicago roll technique to adjust her left side posteriorly. I hear a pop and usually the pain doesn't come back at all or as severely. So even though her left side is "stuck", her right SI joint seems to be the problem because she got a right SI joint steroid shot and has immediate pain relief from the lidocaine and then a week later was having no pain. She got stuck on her left side multiple times over the last few weeks and had no pain which was abnormal from what she has been dealing with. We think the steroid shot is wearing off because she has been getting stuck and feeling the shooting pain again (not a severe as before) so we are thinking inflammation of the right SI joint is the problem and for some reason is causing nerve pain (potentially the iliacus and psoas being atrophied is part of the issue). If she is stuck anteriorly, would self adjusting by lying on her back, rolling into the side position and popping her left SI joint worsen the problem if she is already stuck anteriorly? We are trying to find a way for when she gets stuck and is not with anyone that she can self adjust.

She has an incredibly rare and unique case that has stumped tons of doctors so would love to really talk with you and get your insight into the situation if that is possible.

Thanks for the message and does not sound that rare if honest - a lot of issues can be the right SIJ but you get symptoms on the left side due to compensation - I would also assess her left hip joint to confirm or deny a labral tear of FAI (or both) as that can give you symptoms in thigh and knee. I understand how to correct, my problem is keeping there. I have ligaments that tend to be lax from prior trauma there... I do not want an si belt. Maybe the correction is not being held there long enough at the end phase-. How many times can you do this correction daily, weekly? I was told not to do it very often , & only when I was in pain or the PSIS was more than 1/4 in. higher than the other.
