One thing tapping novices may not realize about these exercises is, by putting them all on the D string, Steve is forcing clean playing. If you are sloppy with either hand, the A or the G (or both) strings will ring out. Obviously, the Satriani 4 finger tapping is a bit different, in that it is on more than 1 string, but the concept of playing cleanly applies even more there.

Shouldn’t you use your middle finger for tapping since the index finger holds the pick.

You can do several things. If I don't have a pick I'll use my index, if I have a pick then I use my middle. I've seen others who play with a pick shift it to under their middle finger for tapping, so what ever is comfortable really 👍
