I really love how compassionate and persuasive Mike’s takes are. He mentions his interest sometimes in principles of logic and debate, and it genuinely really shines especially in videos like this where he manages to come off as sympathetic to any given side of the topic while thoroughly disagreeing with their conclusion and presenting extremely well thought out arguments. I feel like he does a great job modeling professionalism and I’ve learned a ton from this content both in terms of the direct information and like just being a better citizen in a sense

Honestly, I was "blessed" with a very high appetite, I dieted down from 120kg down to 85, now after a couple bulks back up to 100-105 (currently lots of sickness and work stress so I'm not taking it as serious atm), probably a good 10kg of that is fat. While I was bulking I was eating very clean (95%+) and could actually rely on hunger to make me gain weight at .5 pound/week, that's how high my appetite is. While cutting I did 20 min cardio around my workouts, + one 2h cardio session per week (for endurance) and took a leisure walk 30-60 minutes every evening. Just to keep hunger at bay and increase my metabolic rate. This drug would be a blessing and a life saver for me, it would also enable me to work harder towards the part of the sport im passionate about (lifting for hypertrophy), I like endurance work but 20 min sets of liss cardio every day just to maintain gets tiring real quick and isn't fun to me

I spent my entire 20's overweight, ultimately ending up at 525 pounds by the time I was 32. At that point, I started slowly figuring out how to manage my hunger and lose weight, now at 37 I am down to 320 pounds and still dropping. If I could go back and address my issues at 22 with medication and have an actual life instead of spending 6 years learning would I do it? Absolutely.

The learning is the thing. Doctors don't seem to understand that I don't want to learn microbiology. Just tell me what to do, don't give me the tools to go find out myself. I know that doesn't work on some people, but it does for me. Why oh why in a room full of doctors and nutritionists do they always ask me to make the call (besides liability), why do they ask ME to go home and google medicines to decide which one I want? Ugh. I partially love RP because it simply says "eat these many carbs," not "pay attention to how you feel at 35g vs 30g and report back to us." Shut up, plug me into a tricorder and do your job, Doctor! Okay, ;)

As someone on Ozempic I’d like to throw in some anecdotes. 1) yes, eating healthier is important and I have always striven to that but after marriage and having to cook and feed people who refuse to eat healthy food that has been near impossible. I’ve spent a decade gaining weight and declining health in part due to dietary constraints placed on me by others. Having a doctor prescribe me Ozempic has helped this in two ways. The obvious way is that I can eat less and not feel hungry. That’s a huge win. Eating a small bowl of food twice a day and feeling full and happy is a great experience. The other way it has helped is that it served as a wake up call to other people that all my nutritional and medical citations I made about what we eat aren’t wrong because a doctor backed them up. 2) Having a narrow amount of food that I want to eat per day encourages me to eat nutritionally dense food and track my macros. Some of Dr Mike’s books have actually come in handy for that. So thank you. 3) I’ve always had a ravenous hunger that turns into crippling pain when I don’t eat above a certain volume of food. The doctors have never seen anything wrong with this, which has always seemed odd to me. Ozempic has neutralized this. I’m no longer in constant intestinal distress and I devote almost no time worrying about what and when I can eat next to soothe the pain. 4) I’ve always had severe social anxiety, which I think is attributable to hormones. Overnight after starting Ozempic that decreased and has decreased significantly over time. It’s a huge win and has helped me out significantly in my professional life and in the gym because I’m less afraid to ask for help and make contact with people. For the first several months I leaned on it as a crutch to just eat less. Over time the results were enough to help convince myself to push back more for eating healthier and to get others to chime in supporting me, which is something I’ve never managed on my own. That extends beyond family. Friends, co-workers, and acquaintances have all become more supportive of healthier eating habits and lifestyle choices because of the weight they’ve seen me drop. Whereas before they’d almost universally written me off or openly mocked me when I’d discuss why I was as obese as I was. Ozempic has been a new lease on life for me, but that’s my anecdote. I understand that many people will just use it as a quick fix. That’s their right. I hope they use it as a jumping off point for greater lifestyle changes.

Thank you so much for the mature take Dr Mike. I work full time as a Data Scientist at a Hospital at the same time as studying for a Masters. I hit the gym 3 times a week, I swim the other 4 days a week. The ability to work hard isn't an issue for me. But I am overweight because my body is constantly craving food. Even when I'm eating healthily the amount of food it takes to feel satiated (and to be clear I mean satiated, not full and fully realize there is a difference) puts me in a calorie surplus. These drugs have been a gamechanger for me and it's nice to see a more mature, evidence based opinion.

@@kfreps5750 well there are a variety and the video uses ozempic as an example and it's the most common one. I use an app. to track daily calories/macros just to maintain because I know I can't just eat as I want as I basically always feel hungry. My wife is the exact opposite genetically. She does nothing to track what she eats and can maintain a healthy weight without any effort. I am always hungry. I can eat 8-10 plates of food at a buffet where a normie would stop at 4 "feeling stuffed, thought of eating makes me sick." Only by God's will and long-suffering have I made it to the point where this week I shared a Twix candy bar with two other people and that was it (with black coffee, as my pre-workout) and it didn't lead to a hyperhagia episode; I could easily eat a Family/Party size bag of candy bars for Halloween or whatever and still hunger for more food. I'm watching this video (and all of RP's videos) because I want to gain more lean muscle mass so I can live a healthier and more active life. I want to gain more lean Muscle Mass because I recently lost 100 pounds and am no longer obese for the first time since college. I recently lost 100 pounds because I had gastric sleeve surgery. I had gastric sleeve surgery because for some reason the same willpower that got me a PhD, a great career in leadership, and many other huge life achievements wasn't enough to allow me to ignore my hunger. The surgery didn't solve all my problems, and it sure as hell wasn't the easy way out (nor is Ozempic). What the surgery did is reduce both my stomach capacity and my hunger hormones enough to give me the ability and bandwidth to take on a higher class of health problem than simply white-knuckling my steering wheel as I drove past Wendy's on the way home. Problems like whether I can eventually squat my body weight one of these fine days. :-)

I'm convinced type of food has a lot to do with hunger signalling. Years ago I was a live-in Nanny to 3 kids. For 5 days a week I was responsible for their breakfast, lunch and dinner. On the weekends, their (junk-food addicted) Dad took over. I fed the kids mainly home made meals from scratch, nothing fancy but food mostly in its natural form. For lunches I made them wholegrain sandwiches to take to school; protein, some salad veggies, some fruit, and nuts and dried fruit or cheese for snacks. They always came home with energy. On the days their Dad fed them - mostly Maccers for lunch, they were coming to me an hour after lunch complaining of hunger, asking for a sandwich or a banana. I knew what they had eaten was double, two and a half times the calories of the lunches I made them, but they were hungry anyway. Unfortunately I was not allowed to feed them because I was 'off' on weekends (and it was heart-breaking) but their hunger after fast food was apparent, compared to the more natural, higher fibre, lower sugar food I fed them. This highlighted for me how a lot of junk food simply isn't satisfying long enough, and hunger kicks in too soon. So of course, people buy more because they're still hungry.

This week I’m going to get the RP diet app. I’ve always “attempted” to take care of myself best I could, but never researched any details. I was convinced to do the KETO diet, which I was able to stick with for almost two years and decreased my weight from 305 down to 230. I felt myself starting to rebound and couldn’t get back into KETO because (honestly after awhile it just sucks.) As a staple it suited me well, but when carb cravings it was two easy to hit a snickers bar instead of some fruit because I had deprived myself for so long. As part of making my 40s and beyond the best part of my life I wanted to find a responsible way to have a balanced diet and I’m convinced that periodization is the right way. I’m following the steps you’ve outlined. Scheduling my meals and eating Whole Foods consistently for two weeks which has gotten me down to 240, but keto not being a calorie restricted diet (mentally). I’m not sure I could have combated the hunger cravings without the help of semaglutide. So I really appreciate this video in particular. I never struggled with willpower the junk food thing only came after keto. My carb of choice has always been rice and mashed potatoes. But mentally I’ve always over eaten and struggle to feel satiated. Ozempic for me is a crutch to not get discouraged while I’m learning new skills in diet and reinvigorating my enthusiasm for the gym. Honestly I’m all in on the RP culture if that is a thing. I found FitBod before I found RP and already paid for the year, but as soon as that’s up I plan on switching to the hypertrophy app and am currently applying the principles for periodization into the workouts provided by that app. With the information your company gives out for free it’s honestly made FitBod obsolete. At this point it’s mostly just inspiration for tricking myself into progressive overload when my mind says I’m not ready yet. Please continue with the great content.

I just subbed and I'm binge watching. I started my weightloss journey in 2014? I think. I was 325 -330 and struggled horribly with appetite. I would go from under eating to over eating for years trying to figure it out. I'm 250 ..... yeah. Still working through appetite issues but I think I'm finally getting the hang of it. I failed so many times I kinda wish this existed back in 2014. Anyways love your videos. Learning some new information and hopefully buy the end of this year I'll be 220 which was my goal back in 2014.

As someone who has been on Mounjaro for 5 months and just watched this video, Mike is 100% on the money as usual. For me once the food noise disappeared I found myself more drawn to healthier well balanced meals. Am not saying I eat healthier all the time but there is definitely something about lack of food urges and junk food and it is a game changer. I’ve lost 1-2 pounds per week on average, nothing crazy but am now 3 stone down and into a normal bmi range. As Mike suggests am now closing on looking what the next step is, again completely on the money about the increased desire to increase activity and build muscle. If Mike does read this comment, please please can you do a video on his view on next steps as there is a lack of good advice. For me I know I have lost some muscle mass, I know I’ll likely rebound if I come off completely, ideally I want to reduce to a maintenance dose which encompasses a starting foundation of muscle gain without further weight loss. Any advice would be amazing.

I Sped here when I saw Dr Mike has done an opinion piece on the new weightloss drugs. As a layperson in the fitness industry, but with a background in science, I find it particularly daunting just how little scientific material on weightloss and excercise is readily available to laypeople. Its either people who may have some training in fitness but not enough in my opinion for me to feel comfortable trusting their data or thesis, or just straight up misinformation. This was honestly the biggest struggle for me to first start loosing weight and exercising. I will admit, I have been on the fence with my opinion about weightloss drugs, not thar they are applicable to me, but I just hadnt gathered enough informed information to form an opinion yet. I really love these videos and generally hearing an actual proffessor discuss peer reviewed studies and research present his findings and am so glad I stumbled upon you. The argument on "if we had robots do our chores for us, we could could focus our time on higher value add activites" hit home. Heard a story from my girlfriends dad. They asked his grandmother who was born in like 1890 or something what the greatest technological advancement she lived through was. They thought maybe electricity, or improved plumbing or cars etc. Without skipping a beat "indoor heat" was her #1. It immediately solved the heating problem. Prior to this a ton of time in the day was spent collecting wood/fuel, splitting/preparing fuel and stoking the furnace. That ~2-3 hours a day was just given back to everyone and now its completely taken for granted.

I am definitely working on getting my nutrition together; I'm finding healthy recipes that I can meal prep and going to be using the RP Diet App. That said, I still get really hungry and when I do, I'm going to grab what is easiest to grab and quickly eat, which is junk food. I also tend to binge when I get hungry. It's not an ideal situation, but I do feel like it could be a game-changer for me.

Hey Dr Mike :) I have become a loyal subscriber of your channel for about 4 months. I have really enjoyed it and learned so much! thank you!! I have worked out since university (with my on and offs, usually summers off, where I bike and golf, and walk more). I have always struggled with keeping the weight down. It is a huge battle for me and I do believe it is largely my genetics. Lots of people assume I am stuffing three cheeseburgers down my gullet when they are not looking but the simply isn't the case. I would hardly eat anything all day, and I may body would still hang on to fat. I think my body thought it was starving. So after trying everything else I thought Would try your more scientific approach and started your 12 week fat loss plan three months ago and I lost 24 lbs in 12 weeks. that's 2 lbs a week, which I am super stoked about. I have plateaued here at this weight for a month now but I was also kinda taking a break from being as strict as I was the previous three months. I am now on Ozempic and Metformin and I hope they can help me lose another 24 lbs! but I am a little worried it will not work for me, and here is why. It is related to the comments you made regarding watching skinny people eat. I have often noticed that the thin guys, who are sometimes muscular as well (but not always muscular), eat like gorillas and lions and will not gain weight (fat). They will have three huge plate fulls and one dinner, to my one (and they will have desert and I will not). And to add to the torture, they will be a healthy 170-190 lbs (5'11-6-2'2) and I am 260 at 6'3. Our activity levels are also the same, and mine are actually probably higher. This has always seemed to be unfair to me... So I don't think that skinny people just eat less. That may be the case with women, but definitely not with the men I have been around. there is something different going on in their metabolism and genetics. I would say my will power is far greater than theirs too, as I also eat healthier food. I wonder if you could surmise a reason to this?

The bandwidth point is so critical. Im taking semiglutide as i write this. Hunger is like a yawn. You can fight it, and you can win that fight... For a time... But the war is another matter. Calorie dense food calls to me. It is a distraction. Fighting that leaves less patience and bandwidth to do other healthy things. I have the patience to cook now without snacking while i do that. It helps. It really does.

I’ve been a healthy eater (for the most part) and exerciser for 20 years. I am RIGHT on the edge of a healthy and ‘overweight’ BMI and I’m more concerned about weight than I’ve ever been . At 55 (& going through those woman hormone changes 😒) it’s such a struggle just to maintain, let alone lose weight. I tried a semiglutide for 2 months and lost 10 lbs., but also lost muscle and energy. I loved that I didn’t obsess about food, wasn’t hungry and didn’t eat as much, but damn, I was SOOO fatigued all of the time and the lack of energy stressed me out even more because I had terrible work outs or didn’t exercise at all. Sure, the drug is working, but you don’t get the (mental or physical) benefit of real exercise. Anyone who’s been exercising for a long time knows how you’re affected mentally when you DON’T exercise. The drug is like a mental MINDFU#K. As with anything that modifies your body using medical assistance, you sacrifice SOMEthing 😢. I wish I had a Dr. Mike in my life to guide me on where to go next!

I learned how to wrap my foot with athletic tape to allow me to walk (which is part of my job as a land steward for a land trust in Ohio). I needed something to help with my appetite and Ozempic is helping. Reduced hunger, reduced portions, and weight loss. Not to say there haven't been uncomfortable side effects, cuz there have been, but in general manageable. I appreciate your discussion on the topic and will continue along this weight management journey.

What Dr. Mike said at 12:38 & 12:29 is EXACTLY what happened to me. Sure this is anecdotal evidence to support the hypothesis, and I didn't start taking a weight loss drug per-say (more switched to a medication that allowed me to feel my hunger cues again rather than just being bottomless) but after about a year, lowering my portions and losing 30lbs, I suddenly had this flash: like holy shit wait a second I can totally start working out and genuinely, eventually, become lean. So I did! And working out made me want to eat healthier foods so the whole thing snowballed to creating a healthier, happier lifestyle. One thing allowed for me to feel motivated and empowered to make the changes I needed. Without that first step I don't think I'd be where I am today.

My only criticism if that hunger isn't the only reason people eat, they also eat because of anxiety and stress and stuff. Which is, I would hypothesize, the main reason people overeat, not because they're hungry after eating 3000 calories that day. But maybe less hunger sort of helps with that too, in that it makes it clearer for people it's not the hunger that's making them eat cause we just squashed that, and they're still eating too much so they are more likely to go "oh ok, there may be something else here" and spend some energy getting help for that. Other than that the fact we can now fix the hunger part is amazing.

Great points, and I hope we see them become reality in the future. Some points I have noticed within my own family: 1) The Ozempic craze has reached a point that there's a shortage impacting those with diabetes, which is what the drug was designed and marketed to treat. 2) Family members are sharing it among themselves without a prescription or medical guidance. Is that safe? No idea! 3) Family members are...fudging the truth to their doctors hoping to get it prescribed, or to get their insurances to approve it. 4) Apparently it can cause nausea. 5) Anecdotally, the efficacy seems to depend on the injection site. Arm vs stomach, for example. Hopefully, that kind of thing will work itself out as production ramps up and it becomes more accepted. But I'm a little uneasy watching it unfold in the short-term.

I am a frontline primary care clinician in south Mississippi. I would love to hear some commentary on the phenomenon of weight GAIN on max dose semaglutide or tirzepatide. I regularly prescribe these for DM II. The only thing I can imagine is that patients are so confident in the quality weight loss potential of the medication their behaviors significantly worsen or they are eating more calorie dense foods. This is the same phenomenon seen with patients who fail to lose weight after bariatric surgery. I also think a lot of binge eating behaviors go unnoticed. I agree with everything you said in this video. The other issue we face in the south right now is actual supply of GLP-1/GIP RA even though insurance coverage is expanding.

Taking semaglutide not only has helped me with portion control, but I don’t feel pain from being hungry and have more energy that I’ve put into making healthier food as well as getting regular exercise. Almost 3 months in I’ve lost almost 20 lbs and I still have energy and feel great.
