I think is very important to mention this test is positive if your patient stops the extrernal rotation by active pectoralis contraction(due to feeling of the subluxation)

A quick question! While performing the test, can we replace the horizontal abduction to bring the humeral head anterior with placing our hand beneath the posterior humerus and giving it anterior glide instead?

yeah, this one's still missing indeed. Best test for anterior stability is apprehension+Relocation though.

Hey Physiotutors! I have a question regarding the apprehension test. This test can detect anterior shoulder instability. But does it matter if it is atraumatic or traumatic of origin? The paper of Jaggi from 2010 says that a clinical sign of atraumatic structural (Polar type II) is a positive apprehension test. But all the diagnostic accuracy studies I could find included traumatic (with a history of luxation) instability. Is the apprehension test a valid test for diagnosing atraumatic anterior shoulder instability?

Hi Stefan, that's a good point. In this case the test has limited external validity for nontraumatic cases. From a biomechanical standpoint we don't see how it should be any less valid though.

I generally do this test w/ anterior directed pressure to posterior aspect of humeral head - why the HAB instead?

anteriorly directed pressure would be a different test, but makes sense biomechanically. HAB makes more sense arthrokinematically as well.

They simply wouldn’t allow the arm to be moved into much of external rotation and abort the maneuver
