Sir Mike, regarding the Top Missed NCLEX Question in this video (time 33:46), I got confused. The question here is "While the NURSE is OBTAINING the client's signature before surgery, the client states "I do not understand what is included in a low sodium diet after surgery..." You said that the nurse should not obtain the informed consent because this is the doctor's responsibility, right? Nurses should only stand as a witness while the provider is obtaining the informed consent.

Yes they can if it’s mandatory blood testing and as long as it is court ordered . if the physician refuses it can end up in legal lawsuit .

My question, what if you were on a unit and we're assigned to a client, already started checking dox, but we're transferred to another unit and forgot to log out of that doc(not the computer just the documentation) and when logging in again accidentally accessed it, but quickly logged off the document, is it violation or something accidental and won't get you in trouble, asking for a friend. @SimpleNursing
