I was 35 when I had my first PE. I was on birth control pills and I had just driven across the country (and come to find out I have a genetic clotting disorder.) The ONLY thing I felt was sudden onset SOB. Now hindsight, I did have calf pain a week prior, but I attributed it at the time to the hiking I had been doing all over unlevel terrain. I knew immediately what I had, and I chose not to go with EMS to the hospital because 1) our EMS was a volunteer squad and I didn't know who would show up or how long it would take and 2) I knew they would take me to county, who I wouldn't take my worst enemy to. I chose to go to the hospital I had been working at as a nurse extern. But before I left, I did take the aspirin, and I took a couple hits of albuterol from my son's inhaler. I figured I was going to try to get as much oxygen in as possible. When I got to the hospital (normally a 45-minute drive but we made it in 30) my sat was 93. I was very lucky. I had one large clot and several hundred small ones, with all 5 lobes blocked. It was only one year later that I had my second one, and the only symptom was the palpitations. It can hit anyone at any time, and not all of the S&S may be there. If I hadn't been in the medical field, I may have written the SOB off as an oncoming cold or something. The keys with mine were the pain in my calf and the driving across the country. I honestly see so many medics that do not talk to the patients. You can't get that complete picture without getting that history. If you don't know what a med is for, ASK!!!! If you have never heard of the DX they have, ASK!!!! We only look like idiots if we miss something because we didn't ask.

Just had this call on Thursday I believe ….. the patient was a pediatrician with an EKG that looked like an etchasketch and refused to go to the hospital with ALS. Tripod position and sudden onset.

Just tell them that you think the patients might be experiencing a PE, and then tell them the symptoms that you're seeing. Also, go ahead and throw that 12 lead on, you may see a PE on there; although, it usually doesn't pop up on there. The best thing to remember when talking to the hospital is to not overthink it. I hope this helps. Someone may have a better way to tackle this than me but I saw no one has answered you so far so I figured I'd tell you how I'd do it.

Honestly, two huge hints for PE is Shortness of Breath and low sats with Clear Lung Sounds... Anytime you have those two together, possible PE should be at the top of your differential diagnosis list. You tell the hospital those two things and the Doc will want a chest x-ray asap I guarantee it.

I am looking to getting my EMT Cert. I am currently taking an intro to human bio course this semester at a community college. do you think that is enough of a head start or do you think I should take Anatomy and physiology 1 &2 along with a pharmacology course before I start my EMT training?

Can you explain to me why we should give bilateral IV’s? Do you just recommend two IV’s on just P.E’s & Strokes?

Can Aspirin also be used to help break up the clot? There are def better interventions as a Paramedic but just for EMT sake.

From a BLS perspective, if they have chest pains and they are atraumatic you can give aspirin :) But aspirin doesn't break clots, it just makes the blood slippery and prevent further blockages too

There's nothing a Medic can really do for treatment honestly besides giving some oxygen and getting an IV established enroute to the hospital for this type of patient. The patient needs clot busting drugs or surgery to remove those clots, so fast transport to those Doctors that can do that is priority.
