It was obvious right from the beginning where is the weak spot. Why use a simulation based on physical properties of the material, if it will not be used to optimize the design after the simulation showed that it has flaws?

Shell doesn't work. If you use Shell, then an additional layer gets created around circles and you will get errors when using fillets.

Shell can be successfully used if you first extrude the 12mm boss without the circles, select the appropriate faces to shell, THEN extrude the circular features and bores. Fillets will work fine as well with this method.
