I think the automatically angry shopkeepers that you sometimes see are actually the rare fully formed shopkeepers that can spawn from the mothership spawners, these do not make the little icon at the top right show an angry shopkeeper because they are actually a separate entity from regular shopkeepers

Vlad’s is much better than Jetpack in rando IMO. There’s far too much chaos for it to be likely you’ll beat the remaining bosses without taking fire damage.

You need to walk before you can run, try getting through the randomizer. I have 99's and I'm yet to complete it.
