please check this out! I was in a accident, my spinal sac was ripped open, my back was broke in 3 places, I have bulging discs in my neck. After 20 years of being told the leg acheing pain I go through is because of the fore mentioned. I extended my lower jaw one day and warm relief went into my legs, and I also found out when I extend my lower jaw my headaches are relieved. My sinus fluid flows down the back of my throat, pressure on my gums relieved. And I can go #1 and #2 without forcing. Ect... What happens when I don't extend my lower jaw first I feel a cold sensation in my right ear. And then a hot sensation down my neck, then all the tingling starts in my legs stomach, rear end. Sinus on one side of my nose plugs up and a little on the left nostril, head pressure, about 20 to 30 min later not extending my lower jaw my legs are affected and I have told my doctor. If you cut my legs off I would be happy, but not really. This evidence has taking me into a 13 year search with not much info of why. My idea is I'm pinching a artery which causes blood to back up into my head and there's the right nostril thats plugged and who knows what else is getting plugged, I also found out I have celiac disease and imflamation from gluten and just doing stuff. Take methadone 1/4 of a pill every 6 to 8 hours also effector 1 a day. Please help you seem to be the most knowledgeable person I have seen. When my eyes are closed I see fluid flow over my eyes like pouring paint over a ball over and over again. Until I extend my lower jaw it keeps going. And a relief of pressure feeling in the middle upper chest area, every so often. This really bothers me. I was thrown from a truck on the highway in 1994. Plus a lot of other trauma before the accident.

My mother is having some neuro problems im driving her 3hrs 1 way to a hospital at least once a month while watching her mobility and stability decrease. She has t11 lesions along with some high levels on spep and matching o. Bands ....im learning a great deal and have been able to understand conversations with neurologist thanks to you. Just wanted to say thanks.

This is so interesting to learn. I have a spinal cord injury from C4-T5 caused by Transverse Myelitis and have a combination of all of the symptoms. My lesion is not clear cut throughout my cord but in blotchy areas and largely affecting my right side. I became paralyzed in my hands and then right leg in an hour's time. I have since gained much function back after 2 years but still lack fine motor control in my right hand and have various paresthesia in all of my limbs. Despite having to relearn how to walk and use my hands, I will be graduating in a few months with a degree in medical laboratory sciences. I hope some day we can cure and fully treat spinal cord injuries!

can someone explain why you get LMN signs at the level of the lesion and UMN signs below? I don't get it. If the tract is continuous below, wouldn't a lesion interrupt the UMNs and cause UMN lesion at the level of the lesion as well?

what doesnt make sense: in central nerve lesions the crossing of sensory neurons is disrupted. so it should just affect the sensation AT THE LEVEL and not below.

At 13:06 I understood why there is a loss of pain and a loss of temperature sensations due to the lesion of the anterior white commissure in the case of a Central Cord Syndrome. However, I didn't understand why this injury will affect the spinothalamic tract below the level of lesions ? Btw thank you very much for all your videos, I owe you A LOT !!

Hi Guys, does anyone know how a bronchial artery embolisms would cause issues with the spinal anterior spinal artery... Its not tracking in my brain lol?

So bronchial artery embolization therapy is designed to treat patients with severe hemoptysis. So it’s designed to embolize the bronchial arteries by releasing little particles into those vessels to occlude those bronchial vessels that are responsible for this hemoptysis. However sometimes inadvertently those particles released to embolize the bronchial arteries slip into the aortic circulation and then the anterior spinal artery and embolize the anterior spinal artery and lead to anterior spinal artery syndrome. Hope this clears any confusion!

Hey everyone... just wanted to make sure everyone got this... since the bronchial arteries actually drain into the pulmonary veins and that would drain into the la and then lv and then obviously the aorta... hence able to occlude the spinal arteries. Just wanted to make sure everyone got the circulation pathway lol... thanks again ninja nerds!!!

Your patients will be very lucky to have you. Consider going in to teaching later in your career as any highly ranked program would be lucky to have you in its curriculum.

Hey, can someone explain to me why in ACS, the B/L UMN lesions occur below the lesion level at not at the level as well? Zach mentions the collateral going to the afferent neurons at that level, but what good does that do if the whole anterior part of that level is knocked out? There wouldn't be any signals transmitted, isn't it? The same goes for BSS.

Is it possible in complete spinal cord injury to have no sensations and lmn lesion symptoms for atleast 2yrs. Then would it be because of spinal shock phase or lmn lesion in complete spinal cord injury. Pls someone help me with this ques 😖

I have a question here. Why do we have a loss of pain and temp sensation at the level of the lesion in Ant. Cord Syndrome, on the other hand we develop a loss of pain and tem sensation below the lesion in brown sequard syndrom? is it because the posterior horn not involved in the anterior horn? Which means lesion to anterior commisure leads to bilateral lesion at the same level, but lesion to spinothalamic tract leads to an affection 1-2 segments below. i have not been able to understand why!

Horner's syndrome is from the oculomotor never. It is in charge of the autonomic innervation you mentioned not the thoracic spinal cord

In central cord syndrome, wouldn’t the UMNL symptoms show up more in the lower extremities? Since UMNL symptoms show up below the level of the lesion, and the lesion is in the cervical/thoracic segments.. so UMNL presentation shows up below that, in lower limbs..?

naaaahhhh people do not understand this , this topic of spinal cord lesions is considered one of the most difficult part of neuroanatomy say most difficult part of basic sciences . Students who are preparing for usmle part 1 or FCPS part one exam knows exactly how difficult BCQs/ MCqs are from this topic alone ! however this dude has made this topic study so fun to watch and listen and i can bet millions rupees who ever is having a problem in understanding through books can definitely definitely get benefit from this lecture !

My mom got her MRI results back and now she has to go see a neurologist. The primary care stated she has poor blood supply from her spinal cord. She Was diagnosed with cervical spinal injury about 10 years and didn’t mention anything about poor blood supply. but maybe things have changed. Diagnosis Are as follow: thecal sac stenosis & severe foraminal narrowing. I told my mom not to panic. Making appointment tomorrow to see neurologist ASAP and we go from there.

Great show. I am T12 incomplete injury so I'm paralysed from waiste down including bladder n bowel but I have sensations which I try to train to move. X

in Central Cord Syndrome, how can B/L loss of pain/temp/etc. sensations at it's level and below while anterior cord syndrome only loss of sensation at below it's level? both are affecting Anterior Commisura and Spinothalamic Pathway right? am i missing something?

1 quick question, if we have half spinal cord lesion in f.ex. L2 it gives "mixed palsy", will it then be spastic or flaccid ipsilateral monoparesis of lower limbs?
