I dealt with a guy who was really good once. It started with just a $20 and he masterfully distracted and confused me for way too long, changing his mind about what bills he wanted, etc. By the end of it there was around $80 on the counter and the only thing that saved me was remembering that he only started with a $20. For a small breakdown of those who didn't directly follow: He gives $50, gets a pack of gum and $48.91. Balance is 0. He throws in 2 more dollars with the $48 and gives it for the 50 dollar bill. Balance is 0. He gives you the other 50 dollars to get a 100 dollar bill. You've now got his 50 dollars and your 50 dollars in your hands. Which means you owe him just $50, not $100 even though you end up with a total of $100 in your hands.

As a former gas station/convince store cashier. I had this happen way to often. People don't realize how much we pay attention and know... There is way more involved that many will never understand. Good eye and knowledge on the scam.

Its a numbers game for them. Yeah you might be on to them, so were the last 2-3 places they tried it. The next one might not be though. Same reason why they typically target busier places, it increases the chances of you not catching on because you are just trying to keep the line moving.

one simple way ive avoided problems with change is always leaving what the customer gives me in plain sight. i leave the bills they hand me on the counter top until i hand their change to them, so there is never a question what they used to pay.
