I heard from a nose specialist(Medical doctor) that the second most common reason for nasal septal perforations is the use of topics nasal sprays, however it’s still extremely rare even with chronic long term use. I was told that yes nasal sprays after long term use can cause septal perforations and that even short term use of nasal sprays can cause septal perforations, however both instances are rare, even with chronic use. So septal perforations from long term or short term chronic use of nasal sprays is rare. Lastly I was told, intranasal medications can cause septal perforations however this is exceedingly rare, meaning extremely rare. So I don’t know what to think about the safety of nasal sprays with the nasal passage ways. If septal perforation is extremely rare with intranasal medications then I guess I wouldn’t worry about it.

This was an extremely informative video. Even though you don’t have a ton of views on this video it is the most educationally and detailed, informative video that I’ve viewed on the drug. Thank you. To me the long term affects of this medication are questionable. Some studies say it could potentially lead to a degenerative brain disease. I think I will wait for further long-term studies to be conducted and evaluated

Thank you for the compliment! I think that is a very valid point. The package insert for the medication discusses how long term cognitive and memory impairment has been seen in people who have abused ketamine, so I would guess that esketamine could cause those same things to happen.

It is a good question and not determined if it can be used as needed. Maybe it could just be used as a boost when depression gets bad, but currently it was approved as a scheduled medication.

@@DevonCrowell Ketamine and Esketamine is thought to work for depression by allowing brain cells to grow more connections. This strengthening of connections may help the brain to fire correctly and correct depression processes.

It sounds like the 2 hour post observation period continues for every dosing no matter how many weeks the pt. is into therapy. Also is there a position the pt must be and or stay in when the dose is administered? Does the required healthcare setting change as the pt progresses into therapy? For example once pt is into weeks say 4 and beyond does the setting still have to have advanced life support etc. Can a home care health professional visit the home and spend the 2 hours with the pt in the home setting? Yes the 2 hour observation period is for every dose. The patient's head should be tilted back 45 degrees, at least while administering it to keep it in the nose. The last question is a good one, but I don't think that level of detail has been established yet, but it would be nice if it could be in the home setting!

Hi Dan. I'm in the process of trying Spravato. I've been on phone etc. But anyway, still waiting for a center near me that carries my insurance. I only see like 3 places with in a hundred miles and i'm in central Florida! It's been approved for two whole months. I read that you have to take the oral anti depressant simultaneously with it. I'm worried because I can't tolerate side effects of the oral ones. That's why i am resistant to them. I'm currently on Gabapentin, but that's not a anti depressant. So, am i out of luck?

Hi Patrick, it is hard to say since it is still so new. It was only studied and approved to be used in conjunction with an antidepressant medication. You still may be able to get it if your doctor feels comfortable. It would be considered "off-label" and insurance companies may give push back on paying for it. You could look for ketamine clinics as another option.

This drug is expected to come to market in the United States at the end of the month. I unfortunately do not know how long it will take to come to other countries or at what cost. This will probably depend on the laws and regulations of the country that you live in. The drug company (Janssen Pharmaceutical) may be able to answer your questions.

@@Snnjzhzzj It is not a drug that can be taken out of a pharmacy. It must be given in a clinic by a healthcare provider and the patient has to be monitored for 2 hours after the dose. It is expected to be released at the end of the month. The expected cost is $590 and $885 per dose without insurance.
