My MRI looks identical to the one you showed on the video, how do I get I hold of you?

This is such good info. Bruising is key. I wish more doctors would know this equation. Bench press + Loud pop + deformity in the side of the chest + blood down the arm = Pec Major Tendon tear. I went 5 weeks misdiagnosed because the first ortho doc thought it was a bicep tear. Luckly MRI showed everything.

The photo at @5:25 is of me, and I did sign the waiver 23 years ago for it to be used for science and medicine and education. The pec tear happened in either late '98 or early '99 during a max attempt on the bench press, and the surgery took place about a year-and-a-half later in the Fall of 2000. It's still holding up all these years later. I do have to stretch that pec before working out or it cramps sometimes, but one should stretch before working out anyway. I'm very happy with the result of the surgery. If a doc says it can't be fixed, get another opinion. A gen'l orthopedist in North Bay, On did a 30 second visual and said "you can't sew meat." However, I knew it could be fixed because I saw the injury in bodybuilding magazines in the 1980s. I then saw a group in Montreal, Qc, they did an ultrasound and concluded it was a minor muscle tear that would heal itself?! With the obvious deformity and weakness, I remember thinking how are these orthopedists making this conclusion as they're supposed to be experts. A friend of mine had gotten a shoulder surgery from the Sports Medicine Clinic at Carleton U in Ottawa, On, and recommended I go there to see an Orthopedic Sports Surgeon. I'd been seeing gen'l orthopedists up till then. I filled in the questionnaire. A doctor came in with it and he said, "It says you got a pec tear over a year ago and it hasn't even been properly diagnosed. Well, take off your shirt and squeeze your palms together." I did, he took a glance and said, "Yep, that's a pec tear alright, and you'll need surgery to repair it. I'll write up the referral." Dr Allan Liew performed the surgery at Ottawa Hospital. It's important to get that 2nd or 3rd opinion! Before the surgery, I couldn't do a dumbell fly with 10 lbs on that arm, and the whole shoulder felt unstable. If I pushed, I could feel the humerus head shift a bit, then when I'd relax, it would shift back in place and would ache. I believe if I would've shoved hard enough, the shoulder would have dislocated. The second I opened my eyes after the surgery, I knew it was successful as I could feel the tightness across my chest and shoulder. No troubles with the shoulder ever since the surgery. It had been so limiting before the surgery. Thanks for making such a positive difference in the quality of peoples' lives, Dr. Mora.

I am in the EXACT same position as you were. High grade tear of all three heads of the pectoralis major, including the latissimus dorsi attachment of humerus because of the sheer instability. Over time it caused a 360 degree labrum tear. They are trying to do surgery on my shoulder to repair just the labrum and a bicep tenodesis. I was like are you nuts? You want to drill my bicep into my humerus right next to where all this trauma occured from the humeral attachment of the pec tear, where I have periosteal reaction from the sheer amount of trauma over the years. I am losing my mind. I know that the pectoral tears of all 3 heads caused the labrum tear over time. not the other way around. This is madness. I have given up. All i'm told is to see a neurologist after my chest wall MRI.

Unfortunately, It has been several years, probably too long to repair even with this new information. I cant even lean forward or my entire sternum area feels like its collapsing. I want to be euthanized. lol.

Hey doc. How do we contact you for an appointment? I reached out to you few days ago through email. I’m a professional bodybuilder from INDIA competing in the IFBB pro league. I tore my left pec tendon and got it repaired after 2years without any kind of grafts. Post recovery i started training & my strength got back to an extent but the missing anterior axillary fold looks asymmetrical. I consulted a another doc & he suggested me tissue engineering grafts rather than allograft due to the high infection rate. Can you please comment Your thoughts on this?

Great video! I have had pectoralis major repair in 2020 and I wish this video was up by that time as it summarized the whole process and yes i had hematoma on my biceps ( pecs tendon was ripped of humerus ). Recovered full range of motion in 2 weeks of physical therapy and absolutely gained back my strength. However, the pecs shape is still not symmetrical.

I'm 53 years old and I tore mine about a year ago with full intention of getting it repaired immediately. I'm on a blood thinner for mild stroke a few years before my injury and I was concerned about bleeding from bruising internally and I bruised up very badly!! Anyway I went to an orthopedic surgeon and they scheduled me an MRI within a week of injury but for some reason I got an MRI that didn't show what they needed to see and something clerical with scheduling of second MRI a whole month had went by by the time I received proper MRI and by then I had full retraction, doctor was very apologetic but also acted like at my age if it doesn't bother me and I have full range of motion and no real notice of weakness I should consider just letting it be and also because it would be more difficult to repair because of time laps. But it's becoming bothersome because I always feel a mild discomfort in my back and below pec area in ribs due to muscles always compensating for injury plus I do have some cupping and worried about it getting worse over time! I now want it repaired and I have an appointment with same doctor.

I have my surgery next week and they told me from the MRI I had that I have torn the fibres off and not the tendon itself. The doc today said its gonna be a lot harder to attach the stitches and they may come off again! I really want this fixed and want to go back to training eventually as have trained my whole life. (The surgeon doing the surgery is another one I saw 3 weeks ago, who has done lots of these surgeries and was more positive)

Hello Docotor, I have operated on the major tendon of the pectoralis with Achilles allograft, after two and a half months have passed since the rupture. I have been recovering for five months, but I notice the tendon plasty is very tight and still hard, in the armpit area, is it normal? Or is it still early and more time has to pass for the plasty to become softer?

Hello thank you for reaching out. In many cases, the tendon graft remains firm so it needs to be massaged and stretched. But I would recommend that you see your surgeon to discuss and make sure that everything is OK.

I tore my pec and the musculotendinous junction and my ortho encouraged me not to get surgery because there is a possibility that he wouldn't be able to do it. He said it looked like most or all of the tendon was attached to my humorous. Should I try to get him to do the surgery anyway. I am not in much pain and can not really tell from looking at myself that there is any deformity. I play rugby and wish to continue playing as long as I am capable. Will I be able to play for a long time without the surgery and without a high risk of re injury?

Hello, if you don’t have a deformity, it may not be so bad. But I’m sorry that I cannot give you more information than that. Most of my patients come to me because of the deformity. You might want to consult with your physician again to help you with rehabilitation.

I have bruising all the way around my entire midsection. It pooled in my bicep but gravity also pulled the blood down from my chest shoulder area and it collected in my torso. It made my body look like I gained 30lbs of fat in 3 days. Now that fluid pocket is bruising and it’s literally my whole torso( abs side)

This is very great content…. I have a double pec tear that happened to me 7&8 yrs ago I’m only 31 and still active in the gym….. it is very frustrating knowing that I only have half as much power as I did in my teens… I’ve have 2 MRIs and both orthopedics were simply un educated on pec tears …..I would really love to regain the shape of my chest and my strength… I’ve been researching this injury for years now and I think I’d be comfortable with you performing this for me!! Please help me!!

I have been diagnosed with a complete tear at the musculotendinous junction. Surgeon says it’s repairable but due to life obligations and other factors it will be approx. 3 months post injury until I can get the surgery. Can good results still be achieved?

Yes it’s possible. In my practice 3 months isn’t too long. As long as the PM tendon can be re attached it should work.

@@SteveMoraMD Thanks for replying. 1 question before I speak to my surgeon. Can it be repaired by stitching it directly back together or would I be in a situation where a allograft is needed? Or is that dependent on retraction? I’m 43 and at 1.5 months since the injury I have no pain, full ROM and pressing strength is coming back quickly but I am concerned about the next couple of years and how the defect will look. It’s barely noticeable unless I flex my chest. Just don’t want it to get worse. Anyways thanks for your time and what you do. It’s evident you change the lives of your patients ✌🏼

Hello. I have ruptured my left pec major for the second time. This time the surgeon told me it was at the muscle/tendon junction and there was very little tendon left for him to re attach. He said he doesn’t think it will hold. Any advice would be greatly appreciated!

I have a pec major tear from 13 years ago. The doctor at the time convinced me that it would not affect me. LIES! It's been debilitating ever since. I just had an MRI today and am now pushing the issue with a new doctor. Wish me luck!

I tore my pec about 2 years ago bench pressing. I've been a powerlifter all my life and knew this was coming sooner or later. I screwed up by not getting it repaired, I didn't tear the tendon, just the muscle. I was told I didn't need to have it repaired since I told them I wasn't planning on going very heavy ever again. I never stopped bench pressing after I tore it and I recovered well, but there's some feeling of cramp like once in a while during benching. I slowly worked up to 245lbs for 4 sets of 10 reps and today I went up to 275lbs to see how it felt. I didn't feel anything and have plenty of strength still, much more than I figured, but I was told I was never going to go up to 75% of what I used to bench. I've been thinking about getting it fixed to eliminate any potential issues in the future, but I'm also getting older and don't need to use the same weights I have in the past. I don't care what my pec looks like, it isn't that bad. I just wonder every day what it would be like if I did get it fixed.

Hello thank you for taking the time to comment. Its good for people to see that one can regain strength after a PM tear. It’s not the same for everybody. In most cases patients who get it repaired early will have excellent results. Early repair leads to near normal strength. Regardless you are doing really well. Getting it reconstructed at this point may not be worth it especially if you don’t mind the asymmetry. You have adapted well with it.
