The line "I forget my name" is referring to how addiction makes you an entirely different person than who you were before you became so dependent on the drugs. Again, that's just how I interpreted it. Probably because I had a friend who was addicted to coke and became an entirely different person because of it. It completely consumed his life and he was never quite the same after it.
