This information is so useful to clinicians treating patients in pain. We can't ignore the influence of the brain upon pain perception.

What about people that get surgery and improve PAIN & ROM after replacements. Its all about placebo surgery effect?I didn't found any trial about SHAM TKA VS REAL TKA.In wich case we have strong association with structure damage /irritation & Pain? IN OAWe know that OA is progressive ... can we affirm in chronic OA that the tissue heal and its all about sensitization of CNS?If we found sensitization and sensitive/perception distortion in the clinic ? In wich % is affecting the patient and if we tell him that all is in the brain most of eople will get frustrated.i keep following this interest topic... probably virtual reality could give any amazing advance (we talk about perception?)thanks

Doesn't it really just suggest that people have different pain thresholds? How can "my pain is 5 out of 10 today" even be considered a seriously useful tool in terms of formulating scientific hypotheses?

X-ray doesn't show soft tissue. Maybe something there accounts for the difference in pain. To say x-ray doesn't account for difference therefore it must be in the brain is flawed logic.

The X-ray illustrates a significant change in joint space between the 2 which implies changes in structures other than bony tissue.
