I would give an honorable mention to the Last Gasp challenge in s43. The challenge equipment didn’t actually break, but they didn’t consider that the contestants would be able to outlast the tide, even when going completely under water for insane amounts of time. Eventually they just had to end the challenge and give a couple contestants immunity because they totally outlasted the tide and would have had to wait hours for the next high tide before anyone would drop, setting production back. Honourable mention is the final immunity challenge on Guatemala. Being on a wobbly board holding 2 ropes. The board was so wobbly that soon as Jeff said to let go of 1 rope, everyone struggled to the point they were all leaning to the side wall which was legal as long as they didn't touch the side with their hands. Now the challenge pretty much turned into how long can you lean against the side wall rather than how long can you balance on the board.
