It means there is twist in her sobriety. sobriety = not drunk sober. Sobriety can also have a religious meaning, more so especially in some older American klans. Twist = Twist. And the song cannot be translated to mean anything in particular. It is not a song you can apply a universal meaning to. It's a song just to enjoy and delight in.

I have LOVED this song since I was a partying, handsome, self centered young man of 31, that had no sobriety. Fast forward about 35 years to today. I had 21 years of sobriety. went "out" again. Now I'm on my retread 4th year, feeling grateful, and very different!! But I still don't have a clue to what she's getting across! Still love her and the instrumental version!!
