How long of a pause should be between each word? Also, my kid says “more more” instead of just “more”, so does it count if he adds a third word onto that such as “more more milk”?

We also ask him to add a word on like “more milk” or “milk please” but he just says the word we asked him to add on. Do you think this could be indicative of a comprehension issue/him not understanding what we’re asking, or do you think because he’s already said the other word he doesn’t feel the need to say it again with the new add on word?
