The spell seems like it only works when Toji is in the vicinity of strong sorcerers. The grandma wasn't really "strong," but she technically fit the bill so he had to kill her. I think the "instinct" mode kicks in when he sees a strong sorcerer or curse, hence why he was in fight mode with Dagon and Megumi.

For those wondering what the mask does for Ino, it's a way to activate his curse technique that is more of a summoning. He uses the mask to cover his face so the beasts would grant him power mainly acting as the beast itself. But the moment the mask is gone it is only Ino.
