I got my DTV online. Did it myself through the Thailand E-Visa site. Simple process as a remote worker (least for an American). Submit my DL, bank statements, photos, paystub, cover letter on business letterhead was all I needed. No apostilles, police check, medical proof. I did have to pay the nonrefundable costs upfront. Waited two weeks and got my emailed DTV and will be there this month. It is not attached to my passport and that is my only concern. But will see on arrival.

@@thailandinnercircle Did not have to submit a police check but I did show I was a retired police officer so dont know if that helped.
