As for the missing "Trappist 1 A", that is the name of the main star. In star systems we simply name the bodies in the order that we discover them, regardless of their orbit. Since the main star was the first found, it by default is called "A".

Aren't extra-solar planets designated in the order they are discovered? So, Trapist 1 b ist just the one discovered second in this system and can well be the innermost.
