Amy is a clever woman and she accept him just the way he is. Why? Because she can deal with it. No matter what Sheldon says Amy always finds the appropriate answer to make him feel regreat every 'wrong word' he would say. They are both trying to get on well with each other making their relationship get better and better. They finally made it and that's why they got married after all. That's the point.

Like, as an example, I am autistic and I often misunderstand the social cues and say dumb rude stuff that I dont mean to come off that way, but just because I don’t understand the rules doesn’t mean what I’m saying is any less rude. Similarly, a child calling someone fat and ugly may not get that it’s not acceptable or nice to say those things, but that kind of blunt honesty is still rude regardless of intent or lack of understanding. Rudeness in my opinion is more about how words are spoken and received than they are the intent behind them. Not that the intent doesn’t matter, but it is less important to the general perception of rudeness than the delivery and the way it’s received.

When Sheldon acts this way in terms of communication, I don't see much of his behavior as emotional malice toward anyone. But determining if his actions are just his default behavior, emotional disconnection to social norms, or just dislike towards someone is rather difficult. He will jump back and forth for and against you based on changing position on a subject.
