Some advice and tips from me: Best items to get: -Ipecac -Mom's Knife -Any item that grants you wings -Technology + high tear/damage rate -Blue Candle -Book of Revelations or Prayer Card -Obviously any of the rare items like Epic Fetus or Sacred Heart if you can get them, but they spawn less often in hardmode Prioritize higher tear rate over damage if you don't end up getting damage items, so if you get the Cancer or the Curved Horn trinket I would go for Cancer instead. Your red hearts cap out at 16 red hearts, after which you don't get any new ones. I believe the cap for soul hearts is 32, so you can have up to 48 hearts in total. I would skip angel rooms and go for devil room items instead because I never got one to spawn in any of my runs, and you still have a chance of getting angel room items from a Eternal Challenge Room even if you took a devil deal before. Get flight as soon as possible. I believe after taking 7 eternal hearts you have a garantueed chance of getting wings for free, but try to get them before entering the Caves/Catacombs if possible due to certain rooms having shitty enemy placements. If you don't have enough health or a good way to spawn soul hearts past The Depths/Necropolis just find the boss room as soon as possible. If you find arcades enter them and try to get soul hearts off the fortune teller if you get one to spawn. For Mom's Heart/It Lives you want to keep the early enemies alive as long as possible because these are the weakest but also tanky to kill so you can just focus on the boss itself while adjusting to the enemy's projectiles. For Isaac and ??? you want to kill them as far away as you can get while your tears are able to hit the boss. Try to move less and focus on where the tears spawn. The last part can be quite bullshit sometimes due to the angel spawns, tears and homing tears at the same time. Good luck!
