SPOILER: Because of the scene in which Francis reads Caligari's diary, and then the film cuts to a flashback (the whole thing is already a flashback by Francis himself).

I read somewhere that this is actually about a patient perception realization of why he's in an insane asylum. The abstract sets/art Express horror, anxiety & collapse of society overall. As much expressionist art relays. If you think about it, it makes complete sense. What do killers in horror movies represent? Perhaps revenge of someone who's had enough with this society as the Joker seemingly portrays. I know that's not considered a horror film. But to him society is definitely horrible
