Right, like we're going to rallies wearing red nasal cushions chanting "HOME BUTTON.... DIAL..... PRESSURE....HOME BUTTON.... DIAL.... PRESSURE.... HOME BUTTON.... DIAL.... " The person you're referencing is incoherent. If APAP is perfect - or could be "set correctly", it would be acting like CPAP because *there wouldn't be anything for it to react to*. I set my machine to apap occasionally to see if I'm trending up, as a peasant diagnostic tool but just to see what happens. Anyone doing this can see and fathom " that pressure there is stopping hyponeas" - and switch to CPAP. *That* person obviously does not understand the truth in that. If anything, that person may not have the minimum entry level of cognition. Or simply trolling.

Can they be equal? For me, I see no difference between cpap and apap. Sleep like a baby either way! AHI at sleep study 68, less than 2 with c or apap.

Sorry about the Covid -- think I had it last month, too. What a mess! I don't know if I'm just lucky, or that my sleep apnea is so bad -- I suspect the latter, my AHI was 141 i n January -- but I don't have much of any trouble with the apap they gave me after my sleep study. O used to use a fixed pressure machine at 16 for a lot of years, but this AS11 has settled a little over 12, which certainly pleases me (and likely my lungs, as well.) When I got it, it was prescribed with the factory default settings so I had to make adjustments, but I had watched lots of your videos, so no problem. I sleep very well -- anything is an improvement over 141, of course. But if I come up with any problems, I would be happy to ask for a PAP Analysis. Sincerely Your Cult Follower 😂

In my 4.5 years of CPAP therapy, my sleep doctor has __never__used APAP,, but always CPAP with lower starting pressure rising to a fixed nightly pressure.

cutting through all the humor, which let's face it, is half the reason we all come back here ... at 8:00 when you say "he looks good at 13, why not leave it there" makes so much sense. This is really the bottom line on Jason's theory and I like it. Now, all that said and I still turn APAP on but just leave my bottom at 13 which is where i sleep well, but have the option to let APAP raise to 16... just in case i have some leaks or other reasons.

Leaks don't increase pressure... just flow. Pressure can raise from OA, H, and Snore events as well as Flow Limitations, a distortion of the breath waveform tops. The best use of APAP is how you're doing it... once in a great while it can move. Your body responds to steady pressure much better, all that mountain skiing up and down messes with your blood O2 chemistry!

​@scowell Not true, leaks can increase the pressure because they give false positives with Obstructives and Centrals. Especially leak spikes as they alter the pattern. Even when you roll over in bed you typically hold your breath which can trigger an event to raise pressure.

Now this is worthy of a follow up video! What I've seen with this setup are periods where there are movements where the APAP calls it an obstructive apnea and jacks the pressure up...when it was really JUST movement or a post arousal central apnea....again called obstructive apnea. This will often cause a string of central hypopneas or just frequent awakenings from leak or some other unknown reason (I usually attribute it to aerophagia or just general discomfort in speaking with clients) Increasing the mask pressure for leaks wouldn't make sense. The machine will try to target a mask pressure which is independent of the blower speed. In other words, it wouldn't need a higher pressure setting to compensate. Also with leaks...if they're high enough the machine will drop the blower speed to try to reseal the mask. But more times than not it's a mouth leak and that person is already screwed at that point.

Maybe. I really don't know anything about it other than it's miserable. I have seen long term patterns REALLY change post COVID. With one guy he developed severe central apneas that persisted about 3 months and then went away as fast as they showed up. Other than that I think it just generally sucks.

@@Freecpapadvice I think with congestion it makes sense to go to a full face mask temporarily. Do you agree? Or is there a fix? I haven’t thought about. in the past, I tried increasing the pressure to get through the logjam Otherwise, you have to un tape and breathe through your mouth, which sucks for many reasons. For that reason, I always travel with a full face mask back up, and I was glad I had it. I ditched the thing as soon as I started to clear up.

Speaking of cults, I had a "sleep center" assigned to me by default, and when we finally broke up, I realized that my #1 problem, which I repeated at appointments, but never appeared in any notes or reports, was that in four months, I achieved more than 3 hours of sleep with machine-on only twice in four months. And I tweaked everything a human could. SleepMD strongly suggested Cognitive-Behavioral Therapy was the next route to go, and when I told my psychiatrist, he said "Run, don't walk away."

I switched from APAP 5-15 CM, which was my original prescription, to 10 CPAP with VCOM. I use 2 IPR with my nasal mask. If I use my full facemask, I turn the IPR off. I feel better with 11 with full mask.

@@bellarose-au It's a full face mask meaning it covers your nose and mouth, but it goes under your nose not over it. I have the Philips DreamWear full face mask. They aren't as bulky as some of the older masks that completely cover your entire nose and mouth.

@@bellarose-au I'm totally happy with my mask but everyone is different. It takes time to find the right mask for you. I like having a mask that covers my nose and mouth so I don't have to worry about mouth leaks. The Knightsbridge double band chin strap is awesome!! It's helped me keep a good mask seal all night.

@@KPHVAC yeah, masks are a lot of trial and error - at the start especially. My original PAP supplier started me out on the up the nose things, but I couldn't deal with the skin irritation (looked like I had chemical burns) or the amount of air blasting at me. She tried me on a wisp next, and that wasn't much better. I had a lot of contact dermatitis issues with most of the cushions available back then (can't believe it's been 10 years already) and the only mask I could tolerate, using and wearing, was the simplus. Unfortunately, for whatever reason, I'm finding it takes longer to get parts - including the cushion. Which is why I'm trying to change to a different mask. I know my problem with the dreamwear masks (FF and UTN) is the leaking. I think because I'm small and the masks are a bit too big for me, that they leak up and into my eyes which drives me nuts. I'll persevere for a while longer though. Thanks heaps for the insight!

I use APAP but the resmed algorithm sucks balls. My APAP range is on a pretty tight range (3cm) but i still have more things to deal with (like chin tucking). Having s aid that, you'd think that a company like resmed with so much data from CPAP machines could develop a better algorithm or at least use AI, additional sensors to better inform itself as to when it should change pressure settings.

When is the attack planned on the autoASV? That algorithm just outright blows.. my mouth open while giving me horrendous aerophagia. The end result just stinks but does provide relief.

I actually think the algorithm for the ASV works REALLY well if it's used for its intended purpose of Cheyne Stokes Respiration. Problem is that now it's used for UARS or other obstructive events and it just doesn't auto adjust for those things well. Only thing you can do pressure wise is to adjust the EPAP min up. Sometimes the PS settings are set funky as well. Have you tried taping or using a full face mask with it?

As an RPsgT with an Rt background in DME vents and cpaps you know I’m going to titrate myself. Similar to what you suggested I set epap to fixed and PS from 0-5. I’ve only had to go up to 4.2 to get AHI to stay below 5. My AHI on my study was 12. With autoASV it actually made it worse with AHI reaching as high as 21. Actual AI was only 2. So I gave myself more centrals! Yay. We want more! As a cult member will we be getting robes and sandals or the baggy orange scrubs?

Oh and I’ve been using an under the nose full face. The new F40. The really soft cushion is further enhanced by the liquid filled blister on my columella or phyltrum (I think that’s what it’s called) Give me back my Evora.

I think you proved the it was not configured optimally, that auto should have been min 12 or 13 and max 2 above. I always look at APAP the same as me, do I eat the same meal every day, exercise every day, work every day, every day is different so why not allow the machine to adjust a bit every night. As long as it is as close as possible to my optimum treatment pressure it should be fine. I do think that machine makes a bit of difference too. The FP sleep style seems a lot better at this and also heard the Löwenstein machines are better at this as well.

I've seen plenty configured similar to what you're suggesting and it just doesn't play out like that. (I'm not saying for everyone....just the large number of people I consult with) If the machine has a way to F things up...it will. I've done a video on the SleepStyle algorithm. It really wasn't impressive either.

@@cebruthius I don't recall if Löwenstein don't offer heated tubes(haven't really looked deeper in the specs as I was/am still- waiting for a thorough professional review of these machines before considering to buy one)...but yeah, that would be a difference in offered features. However if the air is too cold in your bedroom for breathing comfortable through your CPAP mask....have you tried turning up the heat in your room? 🤔 just a quick suggestion! 😉

I like Pugsy. She was the moderator of my forum too before I closed it down. The only difference of opinion on the board had to do with what content to remove. If it had anything to do with sleep even remotely I wanted it to stay....even if it seemed crazy. I want things discussed openly. That's the scientific method. (even though this isn't exactly a clinical trial)

@@scowell No, your writing was a more aggressive in your face style than funny. I didn't see anyone replying with a LOL to your comment either so I am not alone in my "not funny" opinion. No, I'm not Pugsy. I do have a world of respect for her and believe she does a good job moderating. The forum used to be quite the stinkhole but is a much better place these days. My personal experience does not match Jason's comments. I do not feel more rested with cpap mode than apap mode. Granted, I am using a narrow range (3 cm) but it is still an apap and the algorithm is chasing changes. I'm sure someone could find faulty decisions by the Resmed algorithm but they are not causing poor nights of rest. I'm happy where I am at using apap.

I don’t have the scientific evidence but again I have gotten better sleep in APAP mode than CPAP, with my setting set to 14-16.

@@BLenz-114 CPAP is a single fixed pressure. A narrow range by definition is still APAP and allows the evil Resmed algorithim to adjust pressures. I do equally well on both CPAP and APAP mode based on how I feel after a long, blissful night of rest.

99% of machines we sell are APAPs not CPAP.

APAP is CPAP with an auto adjusting algorithm, it can do static or ranged settings for IPAP.

I really wish docs would start prescribing Bilevel VAutos. I'm definitely not opposed to "autos" for short term use. There are so many that need a change to bilevel it would be nice if it were just a matter of toggling the setting for them instead of having to jump through hoops again.

You lost a lot of my faith also in this video when you recommended the vcom. What a load of BS that device is. You change the airflow change the pressure and then still expect the reading of the machines data to be accurate 😂. Sounda very much like a placebo device to make users think there cpap is more comfortable.

It's not a placebo since it has an effect, but as you say at the cost of messing with the data record. I resent the fact that I have to ask people "is vcom in use" before I interpret their data now.
