I'm not sure if this actually makes any sense, but I interpreted the Reversal Red scene as Toji being knocked back by the force of the Technique since Red still pushes outwards with extreme force regardless if you are hit by the core or not. So, in my mind, the Inverted Spear never hit the core of Red, so it didn't disable it, Toji was just pushed back by the effect of Red. The idea was that the spear couldn't disable the technique if it's just getting hit by the gravity of it.

Proof of that is that Gojo couldn’t heal himself after being sliced in 2 by Sukuna. CE and RCT is apparently produced in the lower belly and that where Sukuna sliced him

The reason why the attack did something to the inverted spear of heaven is because it neutralises any cursed technique but i dont think it works against RCT (Reverse Curse Technique), wich is what Red is, wich would explain why he didnt use any Blue attack in round 2. Btw, he went to heaven because he accepted his death and died thinking about what was actualy important to him, his wife and son, theres also a bunch of other reasons he went to heaven too but i dont remember most of them.
