As a doctor specializing in Medical Nutrition and Metabolic Disorders, I often see patients in my office and from my online sessions with people suffering from metabolic disorders. One of the most important tests that exist to examine metabolic disorders and more specifically carbohydrate metabolism is the sugar-insulin curve.
I believe, and I hope, many of you have heard of the test called the sugar curve. During this test, we administer an amount of sugar, usually 75 grams of glucose in a glass of water, and then measure your blood sugar, or blood glucose, every half hour for 2 to 6 hours. Several of my patients have had this test done in two hours, although in some cases it needs to be done longer, i.e. at least three hours. But this is not it
biggest problem I'm having and that's not why I think almost everyone gets this exam wrong. Because in the vast majority of cases the measurement every half hour for two hours is enough to show us disorders of carbohydrate metabolism.
But another mistake that I see very often is that the opportunity to measure insulin at the same time is lost. Many patients come to me with this test that they have already had and they bring me a reading that is only blood glucose and not insulin. This is another mistake I see often, because it is extremely important that the other person enters the procedure to stay in the laboratory for at least 2 hours, which for all of us is very boring, it is a shame to miss the opportunity to measure insulin at the same time. Only by simultaneously measuring insulin and blood glucose can we get a clear picture of this test.
However, this is not the mistake made by everyone, or at least almost everyone, who takes this exam. The biggest mistake I see happening is not doing proper preparation before the exam. Here I don't know if the mistake is the recommending doctor the examination, the patient who does not listen or does not follow the instructions, or the diagnostic center which should normally ask if proper preparation was done before the examination. However, what is certain is that in the vast majority of cases of people who have brought me results from this exam, the preparation has been wrong.
According to the guidelines, for the three previous days before the exam the examinee must follow a diet that is rich in carbohydrates. This means that he must consume at least 150 grams of carbohydrates per day for the previous three days. But when I ask if the preparation was done correctly, almost always, the person who did the test tells me that they didn't just follow a high-fat diet because they didn't understand the instructions, or because they weren't given the instructions, almost always. followed a low-carbohydrate diet. This means that the test results are no longer reliable.
