What I'm excited about is when my clinicals classes on August 29, 2018. I love home is my cat is waiting for me and my bed. Now that grandpa is home with me and I'm caring for him, I'm excited to come to him too.

I saw on your Instagram stories that you were heading home and I was so happy to see a smile on your face. I am sorry that you still don't feel well. Chronic illness seems so tiring most days. I just came across your channel...this is like watching my life. I totally get every emotion you were feeling. I have lupus, 2 of my kids have autoimmune diseases, and I am a live in caretaker of my dad who is a quadrapalegic with a picc line. It is a crazy life, you are doing great living it as best as you can :)

Hey Mary & Peter! I'm so glad that you can go home this day! I wanted to let you know that I just got discharged from being in the hospital for the past 2 days for my asthma & diabetes. The thing that you will love to know is that when I was leaving the hospital tonight, Mary I was singing your favorite song that you sang tonight. I'm going home, going home, tell the world I'm going home! I was singing that as the CNA wheeled me out in the wheelchair. I automatically thought of you as I was leaving and sang it to my CNA too! I sure hope you Mary are feeling better now too! Prayers!

As a caregiver discharge is completely overwhelming! Our hospital that treats my sons many conditions is 3 hrs away so it’s scary to leave even though we want to go home when inpatient. Coordinating home meds, medical equipment and supplies are stressful!! Glad it all worked out! Praying you feel yourself again soon! High five to Peter too!

Even Olie was excited to go home. Just being at home in your own environment, with your routine, will make you feel a tiny bit better. It is proven people heal faster at home (once it is safe/practical for them to be home). You are so blessed to have someone so devoted to you. You are are both very special people who inspire me a great deal. Thank you for reminding me there is always something to be greatful for.

I totally understand how you are feeling about going home. Whenever I'm in the hospital I cannot wait to go home but it's also scary to be away from all the doctors and nurses. You are lucky to have Peter to help take care of you. I love how he always says WE. Love you guys.

I went through a crazy depression transferring from ICU to a normal room. After such high care I felt abandoned instead of happy. Change of any kind is often stressful.

I'm most excited about clearing out a bunch of boxes full of old equipment my husband was hesitant to throw away. Now I can show him the drawers full of plenty of supplies. He's an always prepared type. Oh, he saves containers too. Yup one of those but I'm so glad for him.

Also, even though you've done 2 weeks of IV's and aggressive airway clearance, your body is probably exhausted and needs rest. I feel like once you're at home you'll be able to properly recover from the hospital because you won't have people coming in every hour to check on you and you can get some rest you still have 1/3 of your IV's to do so hopefully you'll see some encouraging results next week :)

They started a new program a year ago at our hospital where you are assigned a discharge coordinator from the time you are admitted, whether your going to be there overnight or for some undetermined amount of time. This way they are on top of everything that will be needed and in place from the hospital side to the home health care team, to clinic followups, to any specialist referrals and to everything from the paper work to the bandaides they want you to take home. This has alleviated all the gotchas that used to just compound the stress of discharge day while you would normally wait for someone to contact home health and someone else to coordinate the last orders, to someone else getting prescriptions rounded up and then someone else coordinating all the teams signing off and so on. Instead of the family trying to remember that this team said that and that team said this and what one doctor wants you to do, and what another doctor wants you to follow up on, its all put together by your coordinator and they have done all the organization and scheduling, handled all the insurance issues and have everything just set up for you. They come in the afternoon before discharge with any bags of supplies that they have put together that they know you will need at home immediately before home health gets there - or that home health does not have, and a very nice folder with everything organized in it from your doctor's summaries, to their instructions, to when they want to see you again,or what tests they want done, and all the appointments are scheduled and set up, any tests or appointments that need insurance approvals are taken care of. Its all just simply completed. This has meant, for us, that by bedtime the night before the planned discharge everything is in place, anything that still needs to be done at the last minutes is set up and ordered and ready to go so we are not waiting around for anything and whoever is supposed to sign off on anything last minutes is already lined up for that purpose. Also, in place in anything that would need to happen if something last minute blocks the discharge so there is suddenly no issues with a bed already assigned to a new patient and trying to get continuity of care and keeping the same room etc. We know exactly when they plan to be finished up in the morning and they actually give us a time within about 15 minutes of when they will be fine with us walking out the door. Everything is organized and arranged and pretty much all tied up. This is also great for out home health team who then knows within about 15 minutes when we will be leaving the hospital and therefore what time we will be home to meet them. Its taken the most stressful time we seem to experience with being in the hospital, which is getting discharged, out of the hospital, running around getting everything we need and coordinating things to an experience that is almost completely without stress and you end up at home feeling pretty confident that you have what you need and all your support is in place. Your discharge coordinator also follows up that night to make sure that everything she put in place happened and then calls again over the next two days to see if anything more is needed. Its made a world of difference around getting discharged and going home and turned what used to be pretty chaotic and sometimes scary time into a very nicely organized process where you feel like your utterly supported. The reason they started this program is that they found out that, especially with more complicated patients, the rush of discharge and everything happening at the last minute and throwing things at patient and family from multiple teams and doctors to the point where people missed information or got it confused and it was resulting in patients missing follow ups, not getting medications they should and many times ending up with a re-admission within a week as they just were not following up on what they were told to do at discharge. By having someone involved from the start preparing the patient to go home they have removed a huge stumbling block from patient care in that immediate aftermath of discharge.
