Today is July 1st, 2024 and the new national registry (NREMT) written exam is now live for advanced emts and paramedic candidates. In this concise video, I summarize all the updates from NREMT and what you need to know to prepare and pass the NREMT exam for ALS providers.
The NREMT exam is the exam all levels of EMS certification take after they complete and pass an approved educational program. Once the student passes NREMT, they then apply for their state license to practice and start applying for jobs as an EMT/Paramedic provider.
The national registry has four levels of examination, EMR, EMT, AEMT and Paramedic, the new changes in this video are only for the AEMT and Paramedic level exams.

Hey Evan! Took my NREMT last week and I passed! Thank you for these videos and good luck to all the testers. I struggled with signs and symptoms for disease’s and conditions, and that was a big part of the test.

If you took your test and passed before July 1st but did not take the psychomotor or failed the psychomotor. You still are certified at the state level

One heads up for those taking those practice NREMT exams with the new questions - you won’t receive a grade at the end. Not a huge deal, I wanted to see how the questions would look. But I also wanted to see what I got right. You will not be given an opportunity to see this, it’s exactly like the exam. The screen cuts white and you’re done. So, if you do want to grade yourself - make sure to take images/recording of the questions, so you can grade yourself afterwards. If you care, at least. I’m very happy about these changes, I’ve seen and heard of too many medics being forced to take psychomotor multiple times due to pedantic, power tripping, egomaniac proctors.

There are pros and cons to this. My point of view was that the psychomotor exam was a good thing but yeah I felt like it was even a bit subjective when I was in Paramedic school back in the day. Still though there is a benefit to having to actually demonstrate the skills. However, which other healthcare professionals have a similar skill exam? Maybe some do but most do not (other than clinical time) so it is probably more in line with other healthcare providers.

I asked the question too. Surely there's more to it than that. I'm not even sure if the psychomotor tests are like our scenarios, where you just walk in the room after being told "you're dispatched to an 8-year-old who had some peanuts and might be having a reaction" type of question and there are 2-3 assessors and a mannequin. I'm sure American student paramedics have to endure all the assignments, lectures and workshops we endured.

just graduated class in May and registry in early June. It states in the book we are not diagnosing patients yet it sounds like that's what clinical judgement is. Also, What is all this extra learning we have to do for recert? Should I start immediately on recert training or how does it work. maybe a video on that?

Congrats on passing your class and registry! Clinical judgement you should think of it as similar to a field impression. As paramedics we have to conclude what we believe is the next best step in management based on their history, signs and symptoms, events leading up etc, that is how we take action and make decisions with our patients, that is what they are testing you on just like you do in the field. As far as recertification of your national registry, I would first ask your employer if they assist or provide you with recert training. If they don't and it is on you, about 6 months to 1 year before your certification is going to expire you can join a online full recertification program to get all your credit hours in one place.

That I believe has been removed. He mentioned at the start that it is at “all levels”.

Yes, there are no more psychomotor exams for EMT at the NREMT level, you still may be tested in class though.

Can somebody explain to an Australian paramedic what's involved in the training for my American colleagues? Are there standard learning modules to prepare you for these exams? It all seems so different to the training pathway here. The most common way to get your bum on an ambulance seat here is to graduate with a Bachelor of Paramedicine from University. So, as you can imagine, they're full of lectures, pracs, OSCEs etc, plus your assessments by your clinical mentor as you do your internship.

Question for the medics that took the new exam as a recertification not original. Did you find any more difficult? Easier? Better? Worse? I’ll be taking it soon as a non NREMT medic. So technically original but already a certified medic in a non national reg state. Pointers?

Those example questions look way harder than the old test. As someone who’s taken the old style 5 times and has studied hours and hours and spent 100s of $ on test sites. I think this new exam looks too difficult. Don’t think I will ever pass
