Fortunately, you've not had the misfortune of flying with them. Never mind reading about it, experience it. Air travel in Indonesia in the mid 00's was in somewhat of a growth period and the culture and mindset of its people needed a good 10-15 years to catch up. There's an air of "don't worry, let's see how it goes, it'll be fine" amongst most Indonesians, and pilots of a particular generation most definitely had that mindset (I know a few). Let's just say that procedural adherence was just a notional token gesture.

According to the video, the captain decided to fly manually and they changed the mode, which disabled the attitude indicator for 30 seconds. That sounds crazy to me, you lose the AI at the very time you need it most? That surely can't be right. These videos are very poorly explained

RIP, this reminds me of something that was said on an Aircraft Disasters program years ago-that too many pilots rely on the autopilot and have little experience with hand flying. Sounds like an issue with CRM, the FO flies if the Captain wants to fiddle with the controls or vice versa

I have waited for so long for this. Another great video, Allec, for this weekend. Also, yeah, this company throughout its existence is damning. In fact, when the crash happened, it literally drag the entirely of Indonesian aviation industry down (even though it had already had safety issues in the years before this crash), as 6 months after, EU ban all Indonesian airlines from entering EU.

I once fly with Adam Air to my hometown in South Sumatera back in 2005/2006. My plane scheduled to depart at 7 pm, but a technical problems lead to the worst delay i've experienced that forced me to depart the following day at 10 am (albeit they offered me a refund or an overnight stay at a hotel near Soetta Airport, of course as the compensation for my delayed flight)

Pilot training and monitoring in many third-world countries is truly abysmal. Having operated on contracts in many of these countries and witnessed many of these operations I believe that it's only the phenomenal reliability of modern aircraft that ensures that the accident rate isn't far higher. I would like to think that the focus on the accident rates in these countries over the past two decades has resulted in much improved standards, at least, I'm hoping.

IRS devices (Ring laser or Fibre Optic gyros) have no moving parts and, in my experience, are sealed units. So I'm really not sure what a "basic clean" of the IRS would achieve if anything.

I listened to the CVR of the end of this flight. The captain kept saying don’t correct the aircraft it’s on the right heading as it went into a dive, you could hear the wind noise increase dramatically as the planes speed picked up, then you can hear an engine detach from the wing shortly before it crashed

AdamAir was a real shit show, even Garuda Indonesia were never quite that bad (though they had their fair share of fatal crashes)....flying in Indonesia in the late 90s/early 2000s was a bit of a gamble and there were a fair few losses (2 x AdamAir, SilkAir etc.)...thankfully it's much safer now.

Absolutely, & this incident shows that those pilots relied *heavily* on autopilot, & that's a shame if they can't really fly a jet in zero visibility conditions. Like Allec stated, the pilot tried to adjust altitude before leveling the wings which sent the jet into a tight spiral. I'm not a pilot, but I would think recovery techniques like that (leveling wings before adjusting altitude) are *basic* flying skills. To me, this just shows that the pilots over there at that time just weren't properly trained & really weren't instrument-rated.

Always amazes me how many incidents involve the pilots attempting to fix the aircrtaft, so the aircraft can the do the flying, without thinking of flying it themselves and then focus on the problem. Habit I suppose. IFR is always challenging but wings level first and then fix the attitude. Goes against natural instincts. but I had a good instructor.

No It’s the same crash; except the plane involved only had company titles on the fuselage and maybe also orange colored engines but other than that it was in a simple all white livery. All of their other aircraft were painted in company liveries of sorts. (Edit: @9:56 is the photo of the aircraft involved)

In the horrible case here, how could two experienced pilots get too fixated on trying to get their airplane back on course, when they weren't too alarmed about the autopilot being turned off and the airplane was starting to bank. The captain then attempted to assume control of the airplane afterwards when the instrument panel was then shut off, only to get caught into an uncontrollable spiral and crash. It's terrible to notice that Adam Air has had a bad reputation to properly train their pilots and to take short-cuts over safety. How can such an airline be allowed to operate?? RIP to all lost.✝️

If there's no specific livery for that (the one that specific for PK-KKW), the plain white aircraft are used instead. PK-KKW retains plain white, only with Adam Air branding, and engine cover painted in orange.

NY Times (Jan. 30, 2007): Adam Air had a history of safety incidents and the loss of this flight adds to Indonesia's dismal safety record - one of the world's worst. According to several investigators involved in looking at the crash, pilots complained repeatedly about problems with the 17- year-old Boeing 737-400 that crashed. Two of the dozens of complaints were about the plane's weather radar, an item of particular concern since investigators have determined that the plane flew straight into a violent storm before it went down. Normally, pilots would make almost any possible maneuver to avoid flying into the heart of a thunderstorm. By far the largest number of complaints concerned instruments that would tell one of the two crew members whether the plane was going up or down, and whether the plane was maintaining its course. So many complaints, called write-ups, were received from so many pilots that investigators have begun to ask whether any effort was made to repair the problems. The vertical-speed indicator on the left side of the cockpit, the captain's side, collected by far the greatest number of complaints - 48 - in the three months before the crash, the investigators said. The vertical-speed indicator tells how fast the plane is climbing or descending. Pilots complained 30 times about anomalies in the plane's left-right inertial reference system, which helps tell which direction the plane is turning. Problems with a fuel differential light drew 15 complaints. There were numerous complaints about inoperative cockpit instrument lights. There were also several pilot write- ups about wing flaps that stayed stuck at an angle of 25 degrees. On Tuesday, a spokesman for the airline could not be reached to respond to these specific complaints. The company said earlier it maintained and operated its planes according to the guidelines of Indonesian regulators and the manufacturer.
