"To be loved I have to fail" means that some people will feel (consciously or subconsciously) that success will bring punishment, anger, rejection and jealousy (as narcissistic parents are very jealous) so they will underachieve in an attempt to get love and acceptance. This can be underachieving in terms of academics, physical appearance, work or in any other area of life.
