my grandparents could use a reverse tooth fairy. I'll tell them to get a medical degree and say teeth 3 times. Buying a medical degree online will be cheaper than 6 tooth implants.

More that the dentist is threatening using specialist lingo for the worst thing the specialist can imagine. Also doesn't help that he's trying to threaten the human equivalent of charcoal: someone so thoroughly burnt, there's nothing left that's uncharred.

Yep!! Squares with medical coding/ billing, that’s my job. PE’s since 2021 are now performed only on pertinent body areas/ organ systems for that patient, at that visit, according to the provider’s professional judgment to meet medical necessity. So, noting teeth would be unnecessary these days… rest easy, Bill!

If it weren't for an ER resident actually taking the time to look at my teeth, I'd probably be dead. I had a serious infection in the very back top right, which I have no feeling in due to fracturing my jaw inside the socket, damaging the nerve in there when I was 14 and never knowing about it until they were surprised to find 2 teeth in there when they took my wisdom teeth out. If they hadn't taken the time to investigate what looked "weird" back there, they never would've found the massive osteomyelitis in my jaw. I had only gone in because I thought I had a severe sinus infection. Keep on checking them teeth Bill, you just might save someone!

the jaw is safely in doctor territory but yes a holistic approach is helpful and the weird divide we've created between medicine and dentistry probably can cause things to be overlooked

@@subhicshas7553 I do visit the dentist every 3-6 months ever since this happened. I felt no pain in my mouth due to not having any nerves in that part. I really just thought it was a sinus infection. When they took my wisdom teeth out they completely cleaned out the hole and I had x-rays taken about 2 months after showing the bone had grown back into the hole. They assume it got into the bone from my sinus and only found it by looking into my mouth as the swelling had gone downwards instead of out of my face.

When someone asks me what a junior doctor does, my answer is an overglorified secretary - writing notes for other people, calling and taking phone calls on behalf of the consultant, finding documents from decades ago and filling out paperwork. And sometimes, if I'm really lucky, I get to go see someone with low blood pressure and tell them to drink.

As a new NP had a pt come in with “gum sores.” Basically injury from new dentures, no infection. Prescribed some viscous lidocaine to dab on & referral to dentistry. I coded it with a dental related code & had the coder call to yell at me because I can’t use that code. Changed it to an acceptable medical code that was appropriate but caucus. 😂

I have definitely done this before, haha. They stock bread in the theatre tearoom where I work, and sometimes when on call, I only have enough time to grab some bread and eat it plain on the run.

I paid a total of ~$3500 for a couple of X-rays, a filling, root canal, and crown (including the tooth filing, crown manufacture and attachment), on a set of appointments a couple of years ago. And that was a private clinic, not covered by my insurance...

Truth. I had a doctor tell me, "I don't do teeth" as she was treating me for a mouth problem. I stated that the teeth were in the mouth and seemed to be affected by the problem she was treating and received a look that was more effective at shutting my mouth than if she'd told me to shut up. No, medical doctors don't do teeth.

My brother had a really serious infection in his gum and his face swelled up as the infection spread. He went to the ER (it was level 1 trauma center) and the ER docs told him that they couldn't fix it as it's dental work. So he went to the urgent dental office, and they said they couldn't do it because it was oral surgeon's job. Already a couple of days passed and his face swelled up so much that he couldn't see very well. He called orgal surgoens around and they were all booked up. He finally found one- very expensive and cash only- and got that puss removed from his gum and started ABX. It was pretty traumatic experience for him and it kinda shows how the current medical system is not very efficient.

That totally seems like an EMTALA violation. Your brother had a problem and they refused to even give him abx. A kid died from a tooth abscess because the mother was too poor to afford an extraction and the ER refused to treat him. The infection spread to his brain and medicaid ended up paying hundreds of thousands for brain surgery only for the kid to die anyway. That's when Maryland mandated Medicaid has to cover basic dental care for kids.

I cracked a molar on a popcorn kernel once and it ended up getting infected. The pain was so bad I couldn't even think. I went to the emergency room only to get told that while the tooth definitely had to come out, they couldn't do anything about it because it was a dental issue. They didn't even give me pain medication or antibiotics. So, I went home, got a mirror, some needle nose pliers and a bottle of whisky, then performed some old school pirate dentistry. I managed to extract the entire tooth without leaving any bits behind, and the relief was immediate. I went to my regular doctor the next morning and he prescribed antibiotics and told me to never do it again.

@@slook7094 I'm an oral surgery resident and can tell you a lot of hospitals, especially I'm urban areas, have dental/oral surgery on call at hospitals. Can't speak to rural areas where that might be more scarce.

Molars are great and all, but EXTRA molars?? That’s horrifying. Most people don’t have room for all the molars they were born with, can you imagine being given more?! Little crowding in the anterior from a supernumerary tooth or two, nothing many people haven’t lived with. But extra molars truly is cruel. A very just and reasonable penalty for the crime of physicians practicing dentistry, if you ask me.

This makes me feel so much better about the situation I went through recently where my dentist making fun of my family doctor. I went to him(fam) to get antibiotics, not knowing what was wrong with the pounding in my mouth. He told me to make a dentist appointment. Dentist couldn't believe that he didn't know my root was being reabsorbed. I felt so deeply defensive for my doctor. Turns out, he wasn't supposed to; not his job. At least he still sent me to the right place. ♥️ I love my family doctor, he's finally retiring soon(5-7 years) idk what I'm gonna do without him. Had him since birth, I absolutely adore him.

There's really no way to know if a root is being reabsorbed until you take an x-ray and I doubt your family medicine doctor has that kind of equipment

We absolutely do, maxfax surgeons are terrifyingly educated. Had my wisdom teeth extracted by one a couple of years ago (one of them was impacted and needed a little bit of jawbone removed to get it out) and he was like "yes I spent ten years in full time medical training, no I can't use a local anaesthetic it wouldn't be humane, no you can't procrastinate this your teeth are trying to kill you".

I kind of wish medical doctors would just do a super cursory look at the teeth sometimes; my nonverbal super disabled relative was hospitalized and dying in a lot of pain and no one could find the source until a doctor checked the mouth and saw the entire mouth was visibly and obviously rotten and infected. After that it got treated and they're stable and pain free now.

I like Bill's growth as a physician where he was pretty confidently explaining his examination results, prior to mentioning teeth. Show's he's not as anxious and death ridden as he used to be. Or maybe he's just accepted reality and is more chill as a result

From my experience as a dentist, those who are afraid of dentists tend to have to spend very high amount of money for dental treatment due to oral health negligence. Meanwhile normal people just pay for routine check up. Once a year.

As an RD, I would love if attending docs bothered to look past the mucus membranes and check dentition, or lack thereof. I get consults for poor intake and am told by the M.D. (and the RN) that the patient isn't eating well and we don't know why. I walk into the room and in two seconds flat I notice a depressed buccal area. I don't have to ask the patient to open his month to verify he is edentulous, or mostly edentulous. But I do. And then I TALK with the patient about appetite, whether he has dentures with him, difficulty chewimg regular tectures and what textures he can chew. Then I respectfully point out the (obvious) issue to the MD and ask for a dental soft or pureed diet. Geez. Common sense isn't common! As always Doc, thanks for the laughs.

Meanwhile me in Australia. Walk in to a dental clinic (yes, no appointment just walked in). The dentist is like, "What's the problem?" I'm like "My crown broke. It's been a few days. (a couple weeks in reality)" He's like "Ok lets take a look... yeah, mate. It's abscessed. It's gotta come out. We can do it right here. No problem." I'm sensing a trap, how much is it going to cost? And he's like, "I dunno. 100 bucks?" Me, "I'm American, that price is too low. It makes no sense." He takes the tooth out and recommends I get a replacement. I go to pay and the invoice is basically a hundred bucks. I don't believe I could have had a better experience. Getting a tooth extracted could keep an American up at night. Meanwhile, it's just Tuesday in Australia.

Internal Medicine Hospitalist here. I always make the residents give me a report on dentition! Had a lady with bacteremia secondary to severe periodontal and gingival disease last week! Head to Toe exam means exactly that 😀. Keep it rockin' Glaucomflecken!

I was doing my dental internship in a hospital (in Poland) and I loved when they would send the "other" interns with patients to us - they would stand there and be like - you can do that alone? You just fix the teeth here, in your own room?

I have an ancestor who has on their headstone: “Cut Full Third Set Of Teeth.” He also happened to live to 115. 1775-1890. He was my 4th great grandfather. His wife lived to 104.

The funny thing is… I’m a psychiatry resident and we were asked by dental to review patients’ oral cavities on the ward as part of a physical check up. So we can then refer to dentists if and when needed. 😅

As someone with bad crowding, a small mouth, small teeth, and ANCA vasculitis (recent discovery, I was always just told I was a poor brusher/flosser; turns out my immune system is attacking my gums among other things), I can relate to this on a spiritual level.

The sad part of this video is that a person's overall health is directly correlated to how good their teeth are. As a person loses teeth or has dental issues, quality of life notably decreases. You would think more doctors would take an interest in an individual's mouth health. Dentists are actually starting to cross over into the doctor's territory during routine dental cleanings/exams - they look for things in the mouth and throat that might be the start of cancer.

I mean at least in Brazil, it's more like you spend half of your education doing clinics, I have two roommates in odontological school and mainly the latter years of the 5 year program you spend like, 4 hours in theory class, 6 hours doing clinics as part of your class credits. Clinics is basically where the professor puts the whole class to attend the public, usually people who have come through the public system to get their dental treatments, and it's all done under the professor's supervision. You even have different types of clinics for the different specialties of dentistry, basically you spend a semester treating specific types of cases, and, after you finish your degree, you can do a residency to specialize in one of those fields of dentistry which is more clinics, for another 2 to 4 years.

Every time one of them goes into the ER they throw up their hand and say “I know nothing about teeth go see your dentist”. We won’t do eye exams so please, leave the dentition to the professionals

Had a tooth break recently, got it fixed at a new dentist. Best dental experience Ever. 2 days later, same tooth on the Other Side broke. Called the office, "Apparently, the other side of my mouth was jealous...." It was an entertaining call & my appointment is in a few days. As far as I know, none of these people play golf, but my longest-running dentist does, as does his wife. Very glad to have found my new one!

I actually had 5 extra molars for years before they were finally pulled. By that time the extra's were rotten and they were a nightmare to take out and by then I almost needed multiple surgeries to fix the damage they had caused.

I love that the dentist is golfing. I love that he’s like 4 hours late. I’ve never rushed as a dentist. Finally you can tell your an ophthalmologist, because molars are good teeth. They are “the” chewing teeth. All teeth are important.

Please doctors - for the sake of your life and your family's lives, and your patient's lives - take the mouth seriously. Some of my most challenging patients are physicians. I get what you do. My dad was a general surgeon and family practitioner in a small town. He was the original character Texaco Bill was based off of. So find a good dentist. Find one you like. Find one you don't mind seeing 2-4 times a year. Have a relationship with them and the team. It has the potential to change the trajectory of your health and life. And the Reverse Tooth Fairy is actually Kaiser Soze.... shhhhhhhh

Yes please. Because dentistry isn't covered by my provincial Healthcare I couldn't go in due to the cost, a toothache became a tooth abscess became a jaw abscess... By the time I had enough money scrapped together to get it fixed, I'd actually gone to the hospital to see if they could give me an antibiotic which they would not, or call some sort of emergency dentist or something because of the pain, which of course they couldn't. When I finally got to the dentist the freezing of course does not take when your jaw is so badly infected. The dentist gave me a prescription for clindamycin. Because I work with elderly people, little did I know I had long since been exposed to C diff. The infection from that nearly killed me from sepsis. Fortunately that sort of thing can be handled in a hospital and is 100% covered by my provincial insurance. A tooth not being covered by my Healthcare nearly killed me. Dentistry is Healthcare.

@@DembaiVT 100% dentistry is healthcare. We have many patients where a small thing becomes a big thing. In the US many dentists will allow you to make a payment plan. Don't think about insurance etc if it doesn't cover it. It still needs to happen. Good luck!

Funny, but we actually do apply fluoride varnish at our continuity clinic and work side by side with dentists in my residency because our kids frequently don't have a dental home.

Honestly it is an absolute shame that more docs like Bill DON'T talk about teeth. My husband had a heart attack and what we (not being in the medical profession) didn't know was how badly bad teeth contribute to the risk of heart attack. It was his dentist that took one look at his mouth and recognized the main cause. (And when the teeth were pulled, my husband was able to come off several cholesterol?plaque? medications cause his blood work changed drastically afterwards) This (in my totally unprofessional opinion) sounds like a super important factor to me. 😮‍💨😮‍💨

As an acute care Speech-Language Pathologist, I wish more MDs would pay attention to patients’ dentition (and the state of oral cavities in general)… Always baffles me how chlorhexidine gets tossed about and the only oral care so many patients get is just with an oral swab - despite toothbrushes being easily available in every storage area. Anyways, wish I had a Bill on my team who’d listen to my oral care concerns.
