That means the therapist had all three. She was lonely and needed a boyfriend so she created an imaginary woman who undergoes the same tragedy and she is the one trying to help her. Next, she's also an alcoholic and she wants to overcomes that by the help of an imaginary friend's imaginary friend. She also wants to be special enough to become someone else's imaginary friend that she created an imaginary friend's imaginary friend. Lastly, she wants to be a therapist so bad that she imagines her own patients and talks to them about their miseries projecting her own. This was a masterpiece!

Brilliant punchline scene at the end. "The Sketch Show" is a hit and miss comedy. Mostly miss. Yet the characters themselves are talented, and with better material and consistency they would be really good. This sketch is oustandingly inventive and proves my point.
