Carm is in Paris, and has been amazed at the lifestyle, sculputures, etc. Just prior to this scene she is admiring a sculpture on a bridge. Then cut to a gritty New Jersey industrial district, with interstate noise, cars rushing by, horns being blown, and mobster Sil yelling up at a guy cleaning a cartoon strip club sign "Make sure you clean that shit off her tit". Quite a contrast, very sharp script writing...

Tony is not exactly a gay rights activist. What he is pissed off about is losing a loyal subordinate who will do what he is told without question or complaint (unlike Paulie, etc). Vito is also a reliable earner and can't easily be replaced.
