My theory: Hi Five Ghost had human parents, but due to a severe deformity he grew an extra arm on the top of his head, and his brother was born with the same deformity, causing him and his brother dying in the womb. As for his father, he may have had the same deformity, but he miraculously survived and was able to have the extra limb amputated soon after his birth. He didn't tell his wife about the deformity, and they ended up having a kid, who ended up having it. Perhaps when H5G's father died, his body was returned to it's natural original state, either because thats what is naturally supossed to happen when you become a ghost. Or (and this is a reach) maybe God wanted to punish H5G's father for neglecting to inform his wife about his deformity, and dooming his child to die. So, God decided to punish him by returning the extra limb back to him upon his death; The same extra limb he was born with.

I think there are two types of ghosts. Afterlife and species. Afterlife are ghosts who used to be living beings and passed away, species is a kind/race of "ghost-like" creatures, similar to Benson being a "gumball machine" kind/race, and that's what Five is. They have the ability to turn others into "ghosts", or moreso give them the same "ghost species" abilities with no concequenses (they were probs pranking Mordecai and Rigby when they said they would turn into ghosts forever)
