TVD Fan 17 Kai is their uncle. Caroline may have given birth to them, but genetically, they are Jo's daughters. Jo Parker was a witch from their family coven bloodline. Kai is her twin brother. It wouldn't work if Alaric and Caroline actually had sex and she got pregnant, she's a vampire, vampires can't procreate. The coven transferred Jo's kids into Caroline, thus making her give birth to them. So by genetics, Jo and Alaric are Josie and Lizzy's parents, and Kai is their uncle.

That was definitely done for fun in my opinion.
