Well, it might not have been his fault if he was just the pilot. The pilot is not the one doing the maintenance, design, or installation of these types of systems so it makes sense that he could be considered a victim in this as well.

I will never ever be in that situation. I have Zero desire to float in a basket under a flame riding balloon. I am so very sorry for the victims. What a living nightmare. There are no words that adequately express the horror all had to endure, and I am including the spectators. This had to cause PTSD for many.

Having been hot air ballooning twice, it is a fantastic experience. Accidents involving hot balloons are thankfully very few and far between. This tragedy is unprecedented, whoever, whatever is at fault. I can only pity the folks and their families and friends.

I used to crew for a guy who has been flying in the annual Balloon Fiesta in Albuquerque for more than 40 years. Went up twice but no more (vertigo). Our pilot was a stickler for safety and detail (former AF) and we were briefed every time we crewed. All connections were triple checked - hoses, tie offs, basket attachments to the envelope, etc. He never modified anything. This Vancouver pilot wasn’t paying attention as he should have and might have been showboating for his passengers. You never jury-rig equipment. It happens. What a terrible shame for all concerned.

Burning/cooking alive is a most horrible death. Pilot did not maintain his equipment or this would not have happened. The balloon was improperly tethered to the ground and got away ! Complacency by the crew was everywhere .

I don’t know what is considered proper tethering in this context. It was tethered to the trailer, which it actually lifted once it reached the end of the tether during the runaway heating of the air in the ballon, until the strap melted through. 4:42 It sounds to me, and apparently the investigators, that the ground crew did their jobs adequately for a normal flight.

Propane has a specific gravity greater than one. Meaning it is heavier than air and will pool if there is no path for it to flow out. This is why propane cylinders must be carried in a vented locker outside the main hull of a boat. The gas will collect in the lowest part of the boat and patiently wait for a source of ignition. What is surprising is that a wicker basket would not allow the gas to flow through the structure. It maybe the pressure and a continuous flow from the bottle was greater than the flow through the basket. Also, how fireproof was the basket and the materials in the basket? This was horrific! Most boats with fuel for cooking and cabin heat must have an emergency remote cut off to the bottles, if not required by law, certainty by common sense that is if you know and understand the true nature of the gas. A hose with a faulty fitting or a pilot who did not complete his checklist is surely to blame. A small bottle of spray leak detector sprayed on the fittings might have prevented this tragedy. This cannot be unseen in my mind.

I grew up just a few kilometers north from the Langley regional airport. I used to see hot air balloons flying over our municipality, mostly the Remax hot air balloons. Then I realized there wasn't anymore, so I believed this has got to do with banning hot air balloons. Also, Langley and Surrey are southeast of Vancouver.

This video is more than a little confused on the geography. Hazelmere RV Park in South Surrey is 15 KM southwest of the Langley Airport in Langley District Municipality. Hazelmere is also about 45 KM southeast of the Vancouver City Limits. Few people outside of British Columbia have heard of Surrey BC but it does have a population of more than 650,000 and is second most populous city in the province.

I went to school with Jemma, wasn't friends with her directly but everyone knew her from her smile and personality. Horrible tragedy for her family. I haven't had the desire to go on a hot air balloon since.

I remember clearly when I heard the news about this accident. For many years, balloons in the skies of Vancouver were a common sight, and I was planning to go on a ride with my daughter as a birthday gift the following year. I seemed such an idyllic thing to do. Then this happened, and I have not seen a balloon over the city since then. I always wondered what the specific cause of this accident was, and I'm glad I finally found out. It's a pity that safety in air balloons was not taken seriously in this instance. Such "unauthorized modifications" are a definite no-no-NO in general aviation.

Not so simple, or simplistic. Increased efficiency is not necessarily greed, but good business practice, and good use of resources available. The problem is the ‘how.’ Were priorities kept in order within a multifaceted operation? Greed shows itself in deliberate avoidance of things known to need attention for ‘proper’ operation, motivated by having profit be THE, not A, priority, in its place in line.

So likely what Pinnock’s “4th tank” was something known as an inflator tank. Typically used to get the balloon stood up since typically hot air balloon burners go through 2-4 gallons a minute when pulling the trigger. Considering that if a third tank/burner was installed plus extra payload capacity was of concern. It’s likely the thought process behind it. Not defending the man, just a bit of an explanation to the original thought process of the 4th/unknown to regulators tank

He used a separate 10 gallon auxiliary tank for the inflation, and handed it to his ground crewman before the fire according to the report. So the 4th cylinder that was involved in the fire was actually intended to be used for flight.

It just seems odd a small tank would be used in the basket more so than a standard sized tank. Only thing I could think of would be considerations for weight and balance.

In my experience small tanks like the 4th mentioned are to be quickly disconnected once the balloon is stood up and brought to equilibrium with one of the standard tanks turned on. With the “inflator tank” being removed USUALLY in a coordinated effort by pilot and crew or multiple crew members.

Maybe what should be included at the end of this video is a simple discussion of easy ways for peopletrying ballon rides to check that the aircrafts go through routine safety checks by external entities. There's no absolute insurance of safety with anything, but such things seem reasonable and possible safety meansures that might save a few people and raise safety standards in the industry.

I would like more information as to how the pipe disconnected. Was it a crimped sleeve connector, which was being pulled constantly by the ground handling, and eventually the rubber tube pulled out? I can't believe a screwed device would undo itself.

I read the TSB report a few years ago and it said fatigue to the hose from being attached to an outside cylindar (which used only for inflating the envelope and that cylindar was left on the ground to minimize weight) then the hose diconnected from that cylindar and attached to an onboard cylindar for the actual flight. This had to be done each time and likely had to be performed quickly because the envelope was inflated and the craft could be subject to movement if it was windy. After many switches it showed its age and misuse. Could you imagine an airliner with p[assengers on board being managed that way?

I will NEVER travel in a hot air balloon and I did not need this video to come to that conclusion. There was a very good reason why humankind decided that internal combustion was a better way forward when it came to conquering the skies. My external combustion travel experiences were (and will remain always) limited to my childhood travel behind steam engines running on rails. Come to think of it, I will blame my childhood naivety in travelling on a steam powered ferry on numerous occasions. That Ferry was gutted by fire though to be fair it happened while it was docked.

Interesting. With hind sight it is easy to see how this incident could have been much less serious had things been done differently. Accidents will happen so best try and anticipate before things actually do go wrong, aim to make failure less likely and less catastrophic when things eventually do go wrong. After the incident everyone sues everyone else, at least the lawyers got some bucks out this tragedy. RIP.

prob the location of the new tank he added was directly below a burner so it just vented propane directly below igniting when the line disconnected. Could it be foreseen ? idk .. whenever I turn on propane I never open the main valve fully just in case but some people open fully.

10 plus years ago there was an even more horrific ballon accident in New Zealand. Toxicology showed the pilot was a big time stoner and probably high when the accident occurred. Everyone died in this accident, they too were forced to jump from height rather than burn to death.

The Langley airport is NOT in Surrey! It is in Langley! It corners the Fraser Highway to the south, 216th. Street to the east, and 56th. Avenue to the North.

How on earth could you look at 3 propane tanks with emergency cut offs and then install or operate with a fourth tank , without that facility ...would even the most basic intelligence not make you wonder why the manufacturer version had those cut offs but your addition didn't? That exemplifies the indifference that seems to have applied acoss the board here on many operating procedures..

I worked with a gentleman with a single digit hot air balloon licence ( late 1970s ) had many opportunities to fly with him , refused every time . Still believe that one must be stupid to to tickle that dragon .

"Soon to turn fifty, the flight was a birthday gift." And there, my friends, you have a dangling participle. The flight was NOT about to turn fifty soon. The woman TAKING the flight was about to turn fifty. If the flight itself was nearly fifty years old, it would come as little surprise that the balloon was not in the best shape, having been in the air for almost half a century.

What’s that pilot breathing in fire there was nothing he could do seems like he got burned pretty bad you only have a few minutes before the pain comes back I think with the burns the pilot got he did everything he could breathing in a ball of fire isn’t fun
