As I took it he felt real internal conflict and some degree of pain over the loss to come and his gap in support, and jokes are how he deals with both light themes and real problems. I get hospice, I've been through people I care about dealing with those circumstances, and that's as real and as problematic as life gets.

So he is probably talking about his father in hospice in this video.
