You really can't watch Family Guy and take what they say so literally, and look at it more with a satire and irony lens. Peter telling the waiter he's dead to him because he's gay and how him and Lois will focus all their attention to the hostess now isn't a joke about the waiter being gay. It's using satire to show how ridiculous Peter sounds saying that. That saying that to a complete stranger sounds just as ridiculous as when parents say that to their own gay children. Actually they have been canceled loads of times but because the world wasn't so soft they were still able to do the things they do cause is just family joke humor show.
