Stapled hemorhoidctomy causes lifetime disability of fecal urgency as anal cushion (seal) is cut. Most patients cant eat or travel in peace after doing stapled hemorhoidctomy. Kindly educate on function of anal cushion.

Alot of drs think that anal cushion (seal cushion) has no function and do stapled hemorhoictomy and cut the cushion. The function of anal cushion is to seal stools which you should explain as people think only sphincters control continence.

I had surgery to fix external and internal hemorrhoids and rectal prolapse. The surgery was 10 days ago and Im still in crucial pain. Im bed ridden all day and i can barely walk without crucial pain. I just want this pain to go away

18 reasons you should not do stapled hemorhoidctomy for hemorrhoid (piles) patients. (With references) Thesedays Stapled Hemorhoidctomy is strongly being marketed online by big companies and in most videos it looks like piles are being cut with a machine actually they are cutting a circular piece of useful flesh known as anal cushion or hemorrhoidal cushion or hemorrhoidal vascular cushions. 1) Anal Cushion also known as Hemorrhoidal Cushion or seal cushion is cut which is useful material and helps in sealing stools and gives you holding time when you are stuck somewhere. Its like an organ of the body. When you pull stools in with the sphincters the anal cushion fills with blood and seals the stools. The anal cushion is in the anal canal between the anal sphincter and rectum. 2) Pain and bleedind remains for 4 months after stapled hemorhoidctomy with malfunctioning sphincter muscle for 4 months. Most Drs will lie to you and tell you that you will recover in a week or two. Pain is 20 times higher than normal hemorhoidctomy. 3)When the circular flesh (hemorhoidal cushion) is cut with the machine immediately 28 titanium staples of 33mm are placed twice which is 56 staples from the machine to stop the bleeding which is very painful and the body will try to reject and bleeding doesn't stop. 4) The sphincter muscle is pulled up by 33mm (3.3cm) causing tightness / stenosis / stricture, discomfort and damage making it extremely tight.The flexibility, bending and moving space is not there in the anal canal due to tightness. You will be very unhappy and in discomfort. 5) The staples should fall off overtime with the stools but some staples don't fall and remain for years causing problems for which you have to do another procedure called Agraffectomy to remove any retained staples which is costly, inconvinient and which very few surgeons know about. You might have to also cut the scar tissue (surgery area) to bring sphincter down. If they restitch to stop bleeding it can go up again so better to cut with laser. 6) You get life long term side effect such as stenosis / stricture/ tightness of the sphincter muscle causing discomfort. 7) Fecal urgency is another life long side effect. The maximum holding power you will have is 5 min and if you are stuck in traffic you will end up discharging. It will be like a tug of war. 8 ) The Sealing power is gone and therefore you cant hold stools for long time like before so travelling will be hard for you. You have sit at home or not eat much. 9) you can develop anal fissure in the sphincter muscle due to tightness which will give you spasms and force you to do sphincterectomy (cutting of sphicter muscle) which can cause permenant incontinence. In most cases dilatation doesn't work. Fissurectomy (removing external skin to remove tightness) might help. 10) Some Drs think that this procedure is also for External hemorrhoids as when the Sphincter muscle is pulled up the external hemorrhoids tuck in and are not visible from outside and it looks like there is no external hemorrhoid but they are tucked in the sphinchter muscle. 11) Why should you opt for stapled hemorhoidctomy when you have other options like Rubber band ligation and normal cutting of piles (conventional hemorhoidctomy) or laser internal conventional hemorhoidctomy with no long term side effects. 12) Some people have died after this procedure than why should we take the risk. 13) You are damaging your anal canal and its tissues and the flexibility is gone in the anal canal. 14) sphincter becomes dry as there is a reduction in mucos lubricant secreted by mucosa causing dryness and hard stools. 15) It may can cause gangrene (loss of blood circulation) in the anal canal. 16) If the scar tissue heals before all staples have not fallen, the sphincter muscle can remain up and in wrong position and small difference of 1 cm from its original position can cause tightness and discomfort. 17) You are inviting multiple surgeries due to stapled hemorhoidctomy such as agraffectomy, dilatation, fissurectomy, sphincterectomy, anoplasty, botox inj, Stricturoplasty, 18) There is a reduction of blood supply to the sphincter muscle. *** keep in mind that this procedure is for internal hemorhoids only and not for external hemorhoids. If you have external hemorhoids dont do this procedure as lots of Drs think that this procedure is for both internal and external hemorhoids. It makes no difference to the external hemorhoids and have to be cut seperately. Most Drs have been fooled by manufacturers of the stapled hemorhoidoctomy machines. Even if you have internal hemorhoids dont do this procedure. It looks like internal hemorrhoids seem to grow out, detached from the rectal wall, and dangle a little bit? What if they are not detached, and you can't see them unless during a BM, they swell up and bulge...but still not dangly? Are those internal or external? Definitely not on the outer anal area, inside the rectum during non BM's.

Your definition of hemorrhoid is wrong. Another thing of note, I've never seen a case or read a textbook where portal hypertension is a significant risk factor. the distance from the portal system is too long to transmit significant pressure. the term anal cushion is used for blood vessels below both anoderm and mucosa, so it applies to both internal and external. complications that you've left out is fistula. Anemia is not rare; what you mean to say is that it is rarely attributable to hemorrhoids. CBC is done all the time as baseline too.

Yeah I think this guy is confusing hemorrhoids with rectal varices. It's the rectal varices that are the result of portal hypertension, which of course are not the equivalent of hemorrhoids. in fact, hemorrhoisd have been studied to occur with the SAME frequency in cirrhotics w portal htn as the general population.
