After seeing many other specialist I went to a TMJ doctor. Unfortunately the splints that were made kept me awake at night and tend to make me grind even more. Great information! Is it possible to not have TMJ pain, only popping on one side but have terrible pulsatile tinnitus- on the opposite side that gets louder when you apply resistance to jaw? Tinnitus also gets louder pressing on scalp same side and also get facial numbness. Ears constantly stuffy as well. I saw ENT, they did a CTA and everything is normal. No one can offer up an explanation. Thanks

What do you mean by the condyle being remodeled? I think I may have this. I basically don't have that bony nob on the right side of my Jaw unlike my left side. This makes my jaw just hard to control. It feels loose and it gets tired quickly. Sometimes you try to talk and it just does dosent operate like I'm telling it to.

@@zakvince7425 When the disc is displaced without reduction and you are bone on bone the condyle changes shape with time it can become flatter. I have never met a dentist who knows how to treat disc displacements without reduction

@@absolutelynot5xc7789 I know this is a super late reply, but maybe this helps someone. I had the same issues for 2 months (so not for too long). Dentists and jaw surgeons said everything was "fine" and only gave me pills to relax the muscles. Took me 2 months to persuade them to get an MRI scan to find out that my disc was forward and didn't return as it should. Every jaw surgeon said nothing can help me. I got a teeth guard (nothing too special because of where I live there were no expert dentists, my right side bites down and the other side doesn't even touch, not that great). Moved for a weak to a big city and found a physiotherapist tmj specialist and an osteopath also tmj specialist ( so not dentists or jaw surgeons). In just one week there was a loud clic and my muscles feel a lot better. The osteopath said that we got the disc unstuck. Also, my pain and muscle pain and tightness and also my jaw opening, all improved already from just the 1st appointment of osteopathy. Physiotherapy and mouth guards are also essential because these results are temporary if you don't have healthy function and habits and my mouth can lock again. In just one week I feel almost normal. I hope I manage to keep these results. But everyone was telling me that nothing would help which is a lie.

You may want to talk to the dentist that fitted it. It may need to be adjusted. When I was fitted for mine, it took a little time for my jaw to adjust at first. Don’t give up hope. I have been wearing my daytime orthotic for 3 months and am almost pain free, and I was in a LOT of pain to start with. Praying the same outcome for you!

My symptoms got worse after i received a temporary crown. I have had tmj issues in the past but ever since i got prepped for the crown i had a headache on the same side as that crown. I now have the permanent crown in and still having headaches. I have an appointment with a chiropractor to see if that will help.

It took me several months to realize this is TMD pain and now it will be two more weeks before I get my orthotic splint. No, they don't typically need surgery because it's usually the muscles that we just cannot calm down. I'm getting trained in using TekScan (also called T Scan) and I'm hopeful that that will be the solution for at least some of those 10%!

I received Orthotics when i was done wearing braces but i repeatedly got them broken on accident and the waiting period between each time made my TMJ worse. It's located on my left joint mostly and a little at the righ joint. It feels like my jaw is being pushed to one side by the pressure. I've been living with this problem for over 12 years, it messes with my orientation too like i can't tell where my front middle is and i need to focus extra hard to walk straight. Balance is completely off.

So grateful for your information that you bring to us. I do have muscle pain on my right side of jaw which I believe TMJD. I wear a nightguard, but feel an orthotic would be better for me. I have had the nightguard for quite a few years, and the guard has indentations from my bites at night. Will the orthotic help with not only the muscle pain but also from grinding? I am also diagnosed with Trigeminal Neuralgia. In your opinion, is it possible that the jaw pain from TMJ could cause TN?

I have been putting a small piece of cotton in my painful ear with lavender essential oil on it. It works and if I don’t have it in the pain comes back. I take it out at night and it doesn’t bother my hearing at work.

I almost died because i have this and didn't know. I had constant chronic migraines and was taking up to 15 excedrin migraine pills per day. I ended up with 4 ulcers which I'm still recovering from. I'm the only parent of a special needs child. We're both on disability and on public insurance. Almost nobody treats this and nobody at all that takes my insurance. I'm barely hanging on as it is and can in no way afford getting to another state for this. I'm so desperate and in so much pain all the time.

I had custom orthotics made but my jaw was never relaxed with them because my teeth wouldn’t “rest together” right. I would constantly grind on the orthotic and move my teeth around to find a good position to relax in, but couldn’t. Would you say that’s because the orthotic perhaps just wasn’t made right or the right kind, or that I would just need a much longer time wearing it for my mouth to become used to it? It was made after connecting sticky measuring devices to my face to see on a screen where my jaw seemed comfortable most. The orthotics also began shifting my bite, so my bottom teeth would close right under my top teeth, making my smile no longer aligned then. My regular dentist urged me to stop wearing it immediately (the orthotic was made by a neuromuscular dentist).

It’s not about the name or type of splint, it’s about the prescription built into it that is unique to each individual. Kind of like all prescription eyeglasses looking similar but it’s the prescription built into it that makes the difference. 😊🙏🏼

It's hard to say what caused it. There are typically many factors involved. I wouldn't recommend doing jaw exercises as this can make things worse.

In your experience, is it possible for a patient to end up with an open bite or their teeth coming together differently, after wearing these removable orthotics for several months? If so, is there any way to prevent this or minimise the chance of creating long term / permanent changes in occlusion, as a result of orthotic therapy? As you know, some patients cannot financially afford Phase 2 orthodontics and this is why I'm asking this question. Also, why do different TMD dentists use different types of orthotic treatment protocols? Neuromuscular and ICCMO dentists such as you, use the removable day time and night time orthotics which are protrusive and bring the mandible forward and decompress the joint. Orthopedic dentists use a removable single orthotic which the patient must wear 24/7. Some dentists such as your colleague Dr Alex, use a fixed single orthotic which must be worn 24/7. And some dentists recommend stabilisation splints such as the Tanner appliance because they are the most conservative and have the lowest chance of causing any long term changes in occlusion. For patients like me it is very difficult to make an informed decision regarding which type of orthotic is best for us. What is your advice for patients who need to determine which type of orthotic is best for their particular case? If I ask the dentists in my country they never give me any clear answers.

My method - which involves removing the orthotic when eating - minimizes chances of the bite changing. However, there's about a handful of patients per year whose bites change through treatment. For a few of these patients, we can do minor occlusal adjustments at the end of treatment to balance the bite to match the muscles and joints. For the rest, orthodontic or crown work is needed. As for which orthotic is best for you, it truly depends on the severity of your TMJ problem.

For patients with debilitating TMD pain and disc displacement, who have clicking/popping and episodes of the jaw catching/locking, how can these patients eat safely and properly without their day time orthotic? I have all of these same symptoms and eating food is one of the main times that I would need the stability and support of the orthotic and so logically I'm thinking it will be necessary to wear the orthotic when eating? What are your thoughts on this?

I wouldn't say it's the most common sign of TMD, but it can happen yes. I'd definitely recommend seeing a doctor to rule out infection!

Hello Dr. Mistry, How do I know if I need an orthotic or just myofacial release? I don't have clicking or popping, just tight, sore masseter and temporalis muscles. They give me a nonstop "tension headache" affecting the forehead, temples and sometimes down to my left upper jaw and molars. Thx for any guidance you have. G from Ontario Canada

Will orthotics help with arthritis? I went to orofacial pain clinic and they think that’s my cause . My face feels tense around cheek area, neck pain, headaches on forehead . Already schedule an appt with tmj specialist to see if this is the cause or not . Do you have any recommendations in Southern California ? Im between orange and Los Angeles county . I want to explore my options .

Yes an orthotic can still help. Dr John Kim in Irvine comes highly recommended. I don’t know his exact methods of treatment

Hi there! Dental Hygienist turned Doctoral Nurse Practitioner student here. As I am completing a neurology clinical rotation mostly dealing with headaches/migraines I have noticed such a major overlap with my dental career. I wish there were more overlap between dental and medical fields and professionals. I would like to be able to refer more patients to TMD specialists throughout WA. Would you ever suggest an arthroscopy for displaced disc with reduction? And do you have patients that the clicking/popping never goes away even once disc and pain is stabilized?

I don't believe that an arthroscopy can help DDWR long term. Yes, I have some patients where the click/pop remains even after the pain is gone.

if it's a regular nightguard, it likely won't help with pain. if it's a tmj orthotic that's made correctly, it will. the term "splint" can be used to describe a nightguard or an orthotic, so it's hard for me to answer your question.

Do night guards help? I have one from the dentist but no one takes insurance for TMJ in CO and I have spent so much just with tmj consolations. I get headaches dizziness and popping and clicking.

Yes, I recapture the disc with these treatments - sometimes with reduction and sometimes it's fully reduced.
