My guess about the creeper is that it got just close enough to your side for Minecraft to count the damage coming from there, bypassing your shield.

As someone that lags pretty frequently, my understanding of shields is: If your client determines you're not blocking, you will not block an attack. If the server determines you're not blocking, you will not block an attack. And so, every attack against you grants two opportunities for an attack to get around your shield. So it's best to assume that there is a delay between when you start blocking and when blocking comes into effect that's relative to your ping.
