It does not make the phone louder nor does it add any bass - cause that is physically impossible. What those things can do is restrict the sound to one direction while lowering the high pitches. That results in a perceived boost in bass.

In a loudspeaker, the sound is aimed towards the back of the cone, and the sound bounces out. The iPhone sounded different because when Lou inserted it the speakers were pointing straight out the slits of the cone/tube. Different materials absorb more sound, so a toilet paper roll doesn't work that well because the cardboard absorbs sound.
