The medial compartment of the thigh or the adductor compartment is composed of six muscles arranged in three layers.
The anterior layer consists of three muscles.
Originating from the pectineal line of pubic bone and insterting into the pectineal line of the femur is the pectineus muscle.
The next muscle to be indicated is the adductor longus.
This muscle originates from the pubic body just bellow the pubic crest. Its insertion is into the middle third of linea aspera.
The third muscle of the anterior layer is the gracilis muscle.
This muscle originates from the body of pubis and from the ischiopubic ramus. Its insertion is into the upper part of medial surface of tibia.
In the next diagram we will draw the middle layer of adductor compartment . In the middle layer we have only one muscle, the adductor brevis.
This muscle originates from the lower part of body of pubis and inferior ramus of pubis. Its insertion is into the upper part of medial lip of linea aspera. The anterior and posterior divisions of the obturator nerve are passing anterior and posterior to this muscle.
On the third diagram we will indicate the posterior layer of medial compartment of the thigh which consists also of one muscle, the adductor magnus.
The adductor magnus muscle consists of two parts.
The hamstring part originates from the ischial tuberosity. From this point its fibers are directed vertically downwards to be inserted into the adductor tubercle.
The other part of adductor magnus is the adductor part which originates from the ramus of ichium and is inserted by means of a broad aponeurosis into the linea aspera and the medial supracondilar line.
Inferiorly between the two parts of adductor magnus , an opening is present called the adductor hiatus.
At the beginning of the video we’ve mentioned that there are six muscle in the medial compartment of the thigh, but we indicated only 5 five of them . That’s because of obturator externus which is sometimes considered part of the medial compartment of thigh, and sometimes considered part of the gluteal region.
However you should know that this muscle originates from the margins of obturator foramen and external surface of obturator membrane. Its fibers converge and pass posterolateral and upward, and end in a tendon which runs across the back of the neck of the femur and is inserted into the trochanteric fossa of greater trochanter of the femur.
Regarding the inervation of the muscles in the medial compartment of the thigh, keep in mind that all of them are supplied by the obturator nerve with two exceptions. The pectineus muscle which is usually supplied by a branch of femoral nerve and the hamstring part of adductor magnus which is supplied by the tibial part of the sciatic nerve.
