i think specifically the burning basement inclusion was more of a reference to isaac's crumbling dream-state he's in. it was added in the same update that added the flooded caves, dank depths, and scarred womb (all more "destroyed" versions of the base areas) along with the void which is basically an analogy for isaac suffocating to death, and the delirium that that causes in his final moments. the alt floors are a symptom of that deterioration.

4:38 Personally I blame the item description, but here's how Death Cap works. Death Cap is a multiple use active item, that will lower your stats after every use, and change the active item to a slightly more eaten version of death cap, with Issac slowly looking more deranged each use. After enough uses, the active item will be removed from Issac, and you will get a MASSIVE all stats up. also you get poison tears!!! woohoo!!!

Death cap actually gets eaten over time, and when fully eaten it gives you an enormous stat boost and you do keep your poison tears. You should've kept it with you.

Just a piece of advice (out of video context) for when you want to use the R key and when you get the 50.50 chance for Mega Satan: Fight Delirium and complete the whole floor to get as many items as possible and then use the R key (Delirium doesn't force you into the end game cutscene as you have to go into the chest to finish the run). Cool isaac idea: My idea is that we do a tournament with other binding of Isaac KZbinrs. We do community-made challenges. A few ideas of challenges are as follows: Envy mode (every enemy is envy), every item is (blank), Tainted lost runs, among other things. whoever wins gets a bonus for the next round. A few ideas for bonuses are as follows: an extra life, start with a quality 0 to 1 item, one free run restart (only on basement one), among other ideas. I could act as a tournament host, or I could play in it. I have no ideas for the prize for the overall winner. There could be none, for all I care; just being able to talk to you and other Isaac KZbinrs is enough of a pleasure for me.

I think the sun mechanic that made the enemy stronger is referencing icarus in greek mythology, where he flies too close to the sun and it melts the wax on his wings so it clipped and you guess it, he dies And yea, the wax, the wings, the sun, all a neat reference and i really like it
