Top 10 Things I learned Treating COVID ICU Patients | COVID ICU
Some intensive care units in various hospitals throughout this county have designated units for COVID patients. As an intensive care doctor, I've seen many COVID patients in our designated Covid ICU. It's one thing to read about Covid in the medical literature, but seeing actual patients with this disease is another experience altogether. I've learned many things, but I'll focus on the top 10 that stand out to me.

So, starting at the bottom with the number 10 is signs/symptoms. The most common COVID symptoms that I'm seeing are fever, cough, shortness of breath, and body/muscle aches. I haven't seen many patients with the other symptoms we often hear about, such as loss of taste and smell, nausea, and diarrhea. I have not seen any rashes related to COVID, probably because I only see adult patients. I will say that even though confusion and delirium are widespread in the ICU in general, there does seem to be more of that with Covid.

Many COVID patients who require hospitalization have low levels of vitamin D, Which is consistent with what we are seeing in many recent studies that have been coming out. But of course, correlation doesn't necessarily mean causation, so does it just so happen that many patients with moderate or severe COVID happen to have low vitamin D levels? Maybe, maybe not.
And does that mean that we should give every hospitalized patient with Covid big doses of vitamin D when they hit the door? Maybe.
And does that mean people, in general, should supplement with vitamin D? And what is the ideal level of vitamin D for the population, especially when it comes to COVID? Should we target the current general recommendation for everyone, irrespective of COVID, with a goal of 20 mg/ml? Or should we aim for higher, like 30, or perhaps 40? No one knows for sure the answers to these questions. But studies are being done on this. And as we speak, there are 3 RCT for vitamin D and Covid.
Covid is VERY contagious. One of my patients was in the hospital for unrelated reasons. She actually had sepsis due to an infarcted gut, meaning part of her intestine was not getting enough blood flow. It was severe enough to the point that some of the tissue in her intestine had died. When this happens, the bacteria in the intestine can invade the intestine's walls and get into the bloodstream. This is bad news because these bacteria can spread throughout the body, known as sepsis. So besides antibiotics, this is treated with surgery, where the dead gut tissue is removed, meaning part of the intestine is taken out.
And this is what happened to her. And she got better. But after she initially got better, she started having more difficulty breathing. Her oxygen levels were dropping, despite us giving her more oxygen. So we got a CXR and later a CT scan of the chest, which showed bilateral infiltrates, meaning areas of inflammation in both lungs. And this is the pattern we typically see with COVID pneumonia, where it tends to go to the periphery of the lungs and more so at the bottom of the lungs.

My best friend in the whole world just got put on a ventilator today. She has a daughter that was born on the 12th. I wish I could hold her and tell her everything will be ok. She turned 30 in March. Please pray for her. We need her here.

Not good. Her mom asks me to respect her privacy and to not put details on social media. All I can say is if you do your research and look at worst case scenario for having Covid and being put on a vent, its happening to my BFF. If anyone sees this please continue prayers.

@@emilybrooks8763 well my husband survived after 4 weeks of intubation. So if your friend's health is otherwise good, I mean if she is not obese, not a diabetic, doesn't have high blood pressure I bet she has pretty good chances of making it. I hope that will be the case.

My brother Michael died of the virus. He became sick beginning of April, hospitalised on 4th, and passed away on 9th April. He was kind, gentle and the best of us. No chance to say goodbye or tell him he was loved and appreciated.... RIP my lovely brother.

Try to stay calm and think about the best memories of him. We humans are powerless most of the time. My girlfriend got covid on 2days ago. She's been hospitalized. I can't see her, visit her, feeling powerless and weak. I'm praying for her. I know she's a mentally strong girl. I didn't feel the pain of others who are going through this trauma, now I do. I'm feeling like me being changed person.

I am sitting in my hospital room, hoping I am being discharged today. Correlation DOESNT mean causation. I came close 3 times to being put on a ventilator. I am truly blessed to be here, in the moment. Please protect one another.

How are all going? My Mom didn't make it, she passed away because of failed intubation process. Her hearbeat was stopped and CPR was failed due to very low oxygen saturation :(

Yep... My mom died alone in Easter weekend from covid... We could only do a video chat and she was not doing any chatting... She looked terrible and could only blink her eyes to respond... Friday night before she died on Saturday...I called and the nurse held the phone to her ear and I said what I needed to say... No response just the mechanical sound off the respirator...I believe she heard me.... But she was alone for 7 days... Just 7 days it took for her to die ... This is a terrible thing attacking humanity. Be safe.

My sympathies, my mother, Barbara went through the same. Admitted May 7, died May 16. Couldn't visit. FaceTime and the conversation was us talking to her. She could not respond. Thank God the nurse, Lori, was by her side throughout the night until she died. I am thankful someone was there, but bitter that I could not. More bitter that a 9 time convicted criminal could have 4 memorials with thousands present and I couldn't even have a funeral Mass with family.

Thanks so much for the clarity of what to expect if you are infected and making it easy for us layman types to understand. I’ve just been tested and being 74, I’m not too optimistic. My G.P. told me that it’s either COVID or severe influenza. As I suffer from Asthma and Emphysema, naturally I feel that it’s time to day my goodbyes. I’ve really enjoyed your down to earth explanations and at least know what to expect. I’ll be very anxiously waiting for the call saying yes or no tomorrow.

Pray for recovery. My mom went to the hospital for another condition and caught the flu while in the hospital. With her age (mid 80s, and underlying conditions the prognosis did not look good. I prayed often day and night that God would let her come back home and miraculously He did honor the prayers. Hallelujah!

Geez, I can't believe it's been 3 years since the pandemic started. I discovered you around April or May of 2020 and you quickly became my first choice for clinical Covid info, before that I religiously checked my state health dept's Covid dashboard everyday. Hell I still checked it everyday up until a couple months after we in NJ flattened the curve and started on a downward trend. It's weird to revisit some of these older videos, remembering the early days of the pandemic, the confusion, panic and daily fear, especially living in NJ where the East Coast pandemic started. Ever since those earliest days of the pandemic you've been a constant source of positivity and knowledge, empowering us "civilians" to fight against this disease and survive. You are truly one of the silent health heroes Dr. Mike.

Dr. Hansen: You are a young and brilliant doctor and an expert pulmonogist. I have never seen a video before in KZbin like yours explaining the complexities involved in treating Corona victims. I lost my cousin brother a few weeks ago (admitted on August 4th and gone forever on 19th aged 59 who was otherwise healthy) and I could not even have a last glimpse of his face. Trying to understand and searching for why some victims are meeting the unfortunate end at an unbelievable speed lead to your video.

And how do you know it was God that did this it could be biological germ warfare or it could be global warming unearthing germs our bodies are not prepared to fight especially in the age of processed unhealthy foods and lack of proper nutrients and man did that not God . Just because you have issues against God due to your limited mindset doesn't mean you have a right to disturb others about it. Let people to their faith you are not turning us against God . We all know misery loves company but we are not interested in joining your issues against God as we see him in a more favorable light which again is our business. I wanted to share I survived intubation, on the ventilator and put on a coma. I've been in hospital for 6 weeks so far. I will be discharged soon to continue my recovery. There's still a long journey ahead of me for recovery. My country doesn't seem to have physio after I get discharged. I have bilateral severe covid pneumonia. I'm grateful to be alive, and for the team and wonderful nurses who took care of me. Our hospital is on isolation lockdown, which meant I was unable to see any family for 6 weeks even when they were about to intubate me. I can attest to the loneliness. Even though my lungs won't ever be 100% again, I'm trying to keep positive and be grateful for each day. Thank you doctor Mike for sharing these informative videos.

Heroes don't mix politics with medicine. Why are none of them talking about HCQ-HYDROXYCHLOROQUINE? Nothing but the same old Big Pharma putting pressure on everyone through the politicians in their pocket. Most disgusting is KZbin playing politics with people's lives by banning anyone who mentions HCQ-HYDROXYCHLOROQUINE. Can anyone give me a better explanation of why we don't hear a word about it since the FDA approved it for general use after it took an initial beating because the President mentioned it as a cost-effective treatment? I'll wait for an answer...prefedrably from the "No Harm" consortium.

@@capttony6936 "Them" aren't mentioning it because there's no reason to talk about something that's false/incorrect. It's the same reason most people with an influential platform do not tout that the earth is flat - there is NO reason to mention/discuss something if it has already been clarified as false. Hydroxychloroquine is an anti-malarial, meaning its microbe class is "protozoa". C19 is a virus, meaning its microbe class is "viral". Two different microbe classes meaning two different pathomechanisms. Therefore, a drug that works to fight protozoa wouldn't work to fight viruses. It's the same reason antibiotics do not work against viral infections. Listen to actual physicians and scientists, who are required to study science, not politicians. The same person who mentioned and kept promoting HCQ is the same person who mentioned "inserting UV radiation" into a person's blood - zero sense in either of these.

I have work 25 years in the nursing field I was sent to a lot of times I work for the agency and a traveling nurse sent to psychiatric unit at 1 I did not last 6 months only I don't know but something's telling me I don't like it there it's kind of like 18 scary papers all the patience is that you don't know what's going to happen and they follow you and they stare at you I didn't know I got chickened out. But what I love the most is taking care of the seniors no matter how old they are no matter what the condition is and I took care of AIDS patients cancer patients and most of all all my gosh my heart goes to all these kids the oldest is 26 Del testic children in California I handle six of them not a joke that's a handful but I love doing it the right now I can't say I'm done because you know my body still have to work my body still the same it's just my age is 66 and I'm legally blind that's the only thing if I can get my eye back my eyesight back I think the other one that was shut down I will go back to work and give my time and I enjoy working in the Pediatric Nursery I love kiss my mom had 15 I had three but I had nine grandkids thank you doctor for your time thank you for your sharing all this incredible literature but you're sharing to all of us I like it because I just listen to you if I I don't watch television anymore it's so hard for me to see so grateful and appreciate everything you do so take good care of yourself and have a blessed day

It was therapeutic that he acknowledged the extraneous grief when family can’t attend the passing of a loved one. The stress from it can tear a family apart as was the case for me. My mom is covid+ve n is hospitalized. Haven't seen her since 15 days😭 Her oxygen levels had dropped to 78. Now its ok acc to dr.she has moderate covid. She is diabetic n also a heart disease.Hope she gets well soon

Vivek I agree. Hypocrites the father of medicine said, Let your food be your medicine and let your medicine be your food. That is the plan. Put out the worst case scenario. Maintain constant fear in the sheep. We can do this with any disease. You can be made to fear everything.

@@hazelnut0127 Isn't fear mongering he is talking about ICU cases. Covid ICU patients are very very ill. From my own experience, just like stated here, many obese but some very healthy and young patients. Currently we have 27 yo woman who has soon been in intensive care for 2 months.

@@DoctorMikeHansenthe gut plays a big role in one's immune system. please look into gut microbiota and covid 19 possible link and implications. There are some articles by the NIH. I googled "why covid 19 affects the gut" given people are getting stomach and intestinal symptoms. 🙂

Very informative, the best I have heard or read so far. *Thank you* I had a friend in the 70's that was a ER Dr. He was in general practice before and told me he got tired of all the runny nosed kids and the like and moved to critical care. His hobby was a Volvo P1800 and as a Diagnostician for Porsche/Audi I did all the fuel injection and electrical work on his car. He was curious so I allowed him to be present while I made repairs and corrections. He said my job was way more complicated than his. (I seriously doubt that)

As a nurse, I would hazard a guess that a physician carried it from one patient to the next. They are notorious for poor hand hygiene between patients. I've been seeing if for 20 years.

Why don't doctors recommend getting everyone's blood levels of vitamin D checked? Studies show those who don't survive virtually are all deficient. I keep my levels in the upper range and I've never had flu. Nor a useless flu vaccine, and I'm 64.

Thank you so much for doing your channel. I have PAH and have been doing research on how Covid may affect me if I should get it. It's all pretty scary but I'm in close contact with my pulmonologist. I've only been diagnosed for 1 year and continue to learn how to manage this disease according to the weather, diet, and my other diagnosis of multiple myeloma. You need to be the speaker for the CDC... because of your covid briefing I was able to understand my blood work after COVID-19, my own DR was lost!!! He put me on so many meds cause of my blood work, 2.5 months later still no taste no smell and I still get twitching in my head... this virus recks Havoc in your mind body and soul... Get your blood levels of vitamin D checked to make sure you're in the optimal range, not the minimal levels. Take 5,000 mgs vitamin C daily in divided doses, also zinc and resveratrol.

patients should be tested for every nutrient, there are hints that selenium, zinc or iodine may help with covid19. there are studys showing that these minerals have impact on other viral infections. Likely the body uses up many different minerals to fight viral infections in general. Also many of the mineral deficiencys have depression as a symptom. And i am not a big fan of supplements except vitamin D. I would go instead for nutrient rich foods like bone-broth, seaweed, greens, liver, salmon, oysters, shrimps and so on.

Thank you very much Dr. Mike. While watching this video, I realized that I had pneumonia pleuritic chest pain this time last year. The pain was so severe to the extent that I went to the emergency room. The pain started to affect the whole of my right side to the extent that any step I took was worse. They did all the test to see what was happening but they couldn’t find anything. Finally I did a CT scan At Evergreen hospital in Kirkland and that when they found in my lung. Because of how small, the doctors couldn’t determine what it was. I was placed on antibiotics and I took two sets. CT scan saved my life but at the same time put a hole in my pocket.

It's not rocket science. Take 5,000 mgs vitamin C and 4,000 iu vitamin D, and 30 mgs Zinc daily. Iodine, magnesium and Resveratrol are also a good idea. If you contract Covid, increase vitamin C and start taking L-Lysine: 500 mgs in the morning and 1,500 in the afternoon. Increase up to 4,000 mgs if you're a large person or symptoms persist. Do not consume coffee or caffeine. You should be fine after 3 weeks. If symptoms return continue with the regimen until resolved.

0:37 - #10. Signs and symptoms: More common (fever, cough, shortness of breath, body and muscle aches, and confusion), less common (loss of taste and smell, nausea, diarrhea, rash) 1:08 - #9. Low Vitamin D levels 2:12 - #8. This virus is very contagious 3:51 - #7. Patients will feel extreme loneliness (because of number 8!) 4:09 - #6. Treatment: No ultimate treatment is available, however one or more of the followings may be administered: Remdesivir, Convalescent Plasma, Dexamethasone, Therapeutic Doses of blood thinners to avoid blood cloths (Anticoagulation), such as: Lovenox (Enoxamarin, if the patient's kidneys are functioning OK), and Heparin (for damaged kidneys). 6:59 - #5. Testing: It's a guessing game at this point as PCR testing may give false positive (picking up inactive/dead viral RNAs) 7:32 - #4. High BMIs (Body Mass Indexes): Most patients ending up in ICUs are obese or morbidly obese, due to difficulty breathing (e.g., having sleep apnea). 9:03 - #3. Mechanical ventilation: This requires a breathing tube down the patient's throat or performing tracheostomy (puncturing the patient's neck to place a tube into a person's windpipe) and it's unclear who requires. There is no direct correlation between the patient's oxygen levels and needing a mechanical ventilation. 11:01 - #2. Recovery: It depends on the extent of the lung damage due to Acute Respiratory Distress Syndrome (ARDS) due to viral pneumonia or because of inflammation, some may require a couple of days to recover while some may require weeks or months to recover. Some extensive lung damage can be permanent. 13:51 - #1. Unpredictability and variability: So many factors and issues are still unknown about COVID-19. Thank you for this informative video. My father is fighting COVID-19 right now in an ICU in the state of Georgia. They have him in a prone position on a ventilator. We can only see him by video, we are not allowed into the hospital. We are praying SO HARD that he gets better, we love him so much.
