The official cause of PSA182/ Cessna midair, was the failure of both the flight crew of the 727, and ATC. The Cessna was conducting an IFR training flight, with the flight instructor, and student pilot wearing a 'hood' to obscure outside vision, in order to rely mainly on instruments. The Cessna was advised of traffic behind them. ATC warned 182 of other traffic, and a third aircraft- which to this day remains unknown. The 727 crew thought they were clear of the Cessna, and the Captain was heard on the CVR saying he believed the 182 had 'passed off to our right.' The color of the landscape/ homes obscured the small Cessna as they overtook it. THIS accident led to the creation of TCAS.
