Makes lots of sense, Charlie. Funny, my PT has recommended I not pay attention to KZbin videos.. but I have saved yours to my library and though he has helped me through the worst of it, I am ready to continue the work on my own. I like what this video says. Looking back to many years ago when we were all scared to death my dad would never make it through quadruple bypass surgery, he drew us a simple diagram and calmed our fears. Dad was a farmer with only a 9th grade education, but he looked at the science! Thank you for all your videos. Never heard of piriformis syndrome before mine surfaced. Can't get a bew body, but working on a new mindset. ❤

Wow I’m really enjoying your videos. 6 months ago I suffered with Piriformis pain after a 15 mile hilly hike. Tried lots of things - physio, osteopath, acupuncture, stretching and strength etc !! Still have some pain but gosh your videos are so refreshing. I’m hoping I can now fix myself and get back to running again 😊

Good question Robert- I own an over under duplex. Bottom is commercial- top residential… code requires firewall between top and bottom- and so, stairs are chopped!

I have pain for 2 months left butt area and sciatica. The pain always builds up quite quickly during sleep so i awake every 3 hours and have to exercise 30 mins to 1 hour before going back to bed.Always quite sore/stiff in morning and takes a while to get moving. Dont know if its piriformis related?

Are trigger points just neuroplastic pain? Can we get rid of them with just the mindset of there is nothing wrong with me?

May be… but need to look at the entire picture/evidence to better understand IF it is or if it’s structural… that being said, if structural- it appears that most trigger points are neurogenic in origin (meaning driven from irritation of the nerves- for example)… So, for example… if you irritate the L5 nerve… well this could cause referred pain and “tightness” (defined by some as “trigger points”) to the butt or hamstring. The scientist in me though wants to see some studies, like for number one, ok, here's fifty people who had pain longer than six months, this is the change in enviroment initiated, this was the result. I'd just have to add there COULD be a structural defect that is either overlooked or misdiagnosed. But the list is very 'zen' and anybody that knows any eastern medicine wouldn't be surrpised. I started Tai Chi (and salsa) motion classes (except compression) and it makes a world of difference both mentally and physically. That said, my x rays are still two weeks away so it 'could' be something medical, but my first month was a write off and after my second month I'm sleeping on my back again, granted with legs elevated, but the ONLY real pain I get is driving. I can sit in almost any other position, the biggest issues are first thing in the morning, which is brutal, I used to not even be able to put on socks until after a walk and exercises, but thats doable now. Even with driving, its mostly just in the morning that is the problem. So just want to clue anybody in on the validity of strength training and the power of positive thinking. I at first thought it was a herniated disk, but after more 'detective work' its more likely high hamstring tendonopathy. But while I wish there was a more subtle voice from god telling me to get into shape, this sure has done it. Mentally and MOSTLY physically I'm likely in the best shape I've been since I was running, which I may start again but tendonopathy seems common in runners so I may take up a different sport, maybe tai chi or salsa. Everybody says they notice the weight I lost, and since I always had skinny arms and legs, thats all baby fat baby. And if I ever start to slip, just the thought of that pain is pretty good motivation. But I'm older than fifty and have ALWAYS known that the most important exercises seniors can do is strength training, the muscles lose it faster than anything. And I'm in a pretty poor province where most guys my age are blue collar and its amazing the amount of back problems, and even more amazing how many people are NOT treating it with exercise and training but with pills and rest. And with no success.

So long as you don't have any reasons (medically) to NOT take these things... I don't see an issue with it. But is it going to solve your pain- likely not.

If really high pain levels and hard to find relief... might make sense Matt.. to calm things down. Don't be too hard on yourself... just know I wouldn't recommend repeated injections. Cortisone I think is good for like 100 days isn't it? I'd do the DIY video and at least chart out all the pain you feel and where and when so that when you DON"T feel it as much due to cortisone you can still target the problem. I don't have trouble lifting so obviously I'm not in the same boat as you, but most of my exercises, sadly, start with, "some pain is normal when doing this....don't continue it if it STILL hurts two days later". Doesnt help when you are IN it, but actually the strength training mostly stopped things hurting pretty quickly. I can do one leg leg bridges pretty easily now after only about two weeks and have to say that strength training is oddly addictive, you don't really have to wait for the endorphins to kick in, the BIG failing with strength training is your diet, you need to have that fuel in your body. When I first felt pain if somebody said "if I inject you with this it will go away" I'd likely have tried it. I got muscle relaxants and extra strengthe alieve and they quite literally did nothing.

@@mikearchibald744 yeah muscle relaxants and over the counter non-steroidal anti-inflammatories do nothing. I used to do the gym 4 to 5 days a week so I know about the addictive. Over covid I stopped going to the gym so I probably lost muscle

I didn't realize that taking nsaids would impair healing! I thought that the inflammation was bad for healing!

early in the acute pain phase- when the inflammatory process is in overdrive- it may help ease some symptoms… but you don’t want to stop the inflammatory process- it happens for a reason- to do what is needed to repair your body!
